Job overview

The Dementia Navigator will work in an integrated manner as part of the City and Hackney Dementia Service multidisciplinary team. Their primary goal is to enhance the identification, engagement, and support of service users, carers, and families impacted by dementia. This collaborative approach will enable individuals to live well within the community for as long as possible. The Navigator will also collaborate with other local organisations and agencies to provide comprehensive support for dementia patients and their families.

The Navigator will offer detailed information, support, and advice following a dementia diagnosis. They will engage patients to facilitate timely diagnosis, guide them through the screening and diagnostic processes, and ensure that individuals and families are informed about the support available to them.

Additionally, Dementia Navigators will participate in joint meetings and events organized by voluntary care sector enterprises. In these settings, they will provide information on dementia, promote the service and its benefits, and encourage and facilitate referrals.

Main duties of the job

Maintain and effectively manage a caseload, providing action plans for individuals affected by dementia. These plans will help them adapt to the changes they experience and ensure they receive appropriate support tailored to their evolving needs.

Provide structured, ongoing support and follow-up reviews, adjusting support plans as needed based on the unique needs of each person affected by dementia.

Complete risk stratification tools to identify ongoing needs, interventions, and risk management strategies for each individual.

Maintain accurate and timely patient records, both written and electronic, in accordance with Trust and service line policies and procedures, under supervision.

Work within a multidisciplinary team, following support plans and reporting to senior staff. This may involve assessing, planning, and implementing interventions in collaboration with service users, carers, and other services, considering the diverse needs of the community.

Be mindful of the psychological, social, physical, recreational, and spiritual needs of all service users. Recognise and value service users as individuals, always upholding their dignity. Ensure that care is respectful, inclusive, and supportive of service users and their carers.

Encourage service users to take an active role in their care, promoting their autonomy and ensuring they are involved in their care decisions, with their consent.

Education/ Qualification/ Training

Essential criteria
  • Required to have NVQ3 plus additional training to diploma level equivalent, or equivalent combination of short courses/experience in Dementia Care
  • Experience of working in a Dementia Service / organisation for at least 12 months

Experience

Essential criteria
  • Experience of working with service users with dementia
  • Experience of working with statutory and voluntary agencies is an advantage, but not necessary
  • Experience of managing volunteers
  • Experience of record keeping and writing reports

Knowledge and Skills

Essential criteria
  • Effective communication skills, both verbal and written, including the ability to communicate with confused older people.
  • Ability to work without direct supervision and liaise with external agencies as required.
  • The ability to work well under pressure.
  • Ability to motivate others and work as a team
  • The ability to be flexible in terms of changes to clients visited, visiting times, and tasks to be performed on visits.
  • A patient and diplomatic manner when communicating with clients and their relatives.
  • Ability to manage caselod
  • Ability to complete Risk Stratification Tools / Risk management form
  • Lone working experience
  • Ability to signpost to relevant agencies e.g. adult social care
