Job overview
We are recruiting two Band 4 Patient Pathway Coordinators to join our Patient Administration Team. These roles are ideal for individuals with strong RTT and eRS experience who are committed to delivering high‑quality patient pathway management.
Working closely with clinical and administrative teams, the postholders will ensure that patient bookings, clinic management tasks, and eRS worklists are completed accurately, efficiently, and in line with Trust standards.
Main duties of the job
Key duites include:
- Monitor and review patient care and/or treatment progress using the Patient Tracking List.
- Identify from the Tracking List the next steps in the patient care pathway in line with clinical need (peer review).
- Receive patient referrals and administer in line with timings and requirements as set out in Trust and National policies
- Provide day-to-day supervision and oversight of duties for the department’s administrators.
- Providing support for day-to-day queries from administrators.
- Responsible for collecting and updating patient information on the relevant databases in line with local policy,
