An exciting opportunity has arisen for dynamic, motivated and innovative Pharmacists to join our expanding Medicines Management team at Central London Community Healthcare Trust.

We are looking for pharmacists to join our established and experienced team to enhance the service provided. You will be joining a team with opportunities to implement your skills and knowledge and develop them further.

We are looking for a highly motivated, enthusiastic, reliable Pharmacist who is able to lead and support the Herts Medicines Management Team – to be flexible, plan and organise own workload aswell as developing and supporting the junior staff.

Main duties of the job

If necessary, depending on development and service needs, the post holder may need to work in different localities Trust wide as part of an integrated team to ensure high standards are maintained within Community Health Services and Bedded Services.

The post holder will support medicines management governance work streams in reviewing policies and procedures and to review risk management strategy aswell as providing professional expertise, advice on governance and patient safety whilst supporting the delivery of a high quality clinical pharmacy service and medicines optimisation service to patients across services within Herts.

There is no weekend working and oncall cover is provided remotely.

The post will require travelling between CLCH sites covered by the Herts Medicines Management team so please check the geographical coverage of the organisation before applying.  Flexibility will be required to travel to different sites sometimes within London and as the organisation continues to grow.  A car would be useful for travelling.

Detailed job description and main responsibilities

  • To provide professional expertise, advice, support and training on policies and practice to health care professionals across the Trust.
  • To contribute to the clinical governance of the Trust through interventions which optimise the management of medicines in all relevant areas of practice.
  • To support and deliver a high-quality clinical pharmacy service and medicines optimisation service to patients in Community Hospitals and ensure an integrated approach to the management of medicines and continuity of pharmaceutical care across the interface
  • To lead, manage and develop staff within the team

Person specification

Education/Qualification

Essential criteria
  • Vocational Masters Degree in Pharmacy
  • Professional registration with the General Pharmaceutical Council (GPhC).
  • Postgraduate Clinical Pharmacy Certificate qualification or equivalent
  • Postgraduate Clinical Pharmacy Diploma qualification or equivalent
  • Takes personal responsibility for Continuing Professional Development as evidenced by portfolio
  • Uses reflective practice for personal and professional development
Desirable criteria
  • Independent Prescriber or working towards qualification

Experience

Essential criteria
  • Post-qualification experience of clinical pharmacy, medicines information and prescribing advice
  • Experience of working in secondary care and in a multidisciplinary environment
  • Experience of providing a Clinical Pharmacy Service with good medicines management initiatives
  • Experience of working effectively within multi-disciplinary groups
  • Experience of undertaking structured medicine reviews
  • Experience of training and tutoring in a hospital setting
  • Experience of audit techniques
  • Experience in supervising a team / service
Desirable criteria
  • Primary care experience including Community Services
  • Experience of line management
  • Experience of project management
