West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Join the Integrated Community Response Service and play a vital role in providing out of hospital care and enabling people to receive acute care at home rather than in hospital.  This is a new post within the service, and we are seeking a dynamic paramedic with a genuine interest in working within a community multi-disciplinary team.  An element of the job will be to work with the clinical leadership team to fully develop the paramedic role within the team. 

 

The Integrated Community Response Service (ICRS) provides care for adults in Hounslow via our Urgent Community Response, Hospital at Home and Complex Discharge (Home First) pathways.  There will be an opportunity to work across all these pathways and to build relationships with referrers including London Ambulance Service and primary care paramedics. Clinically the successful postholder will be supported by experienced clinicians including GPs, nurses and other allied health professionals.  They will also be supported to gain appropriate clinical supervision and development.

 

The postholder will be required to contribute to the service staff roster from 8am to 8pm; seven days per week on a shift basis (more details can be found in the recruitment pack)

Provide high-quality care to patients in the community who are experiencing an acute deterioration in their health, which could otherwise lead to a hospital admission. Urgent Community Response paramedics assess, manage, and treat patients in their own homes or care settings, aiming to review patients within two hours of referral. As part of a multi-disciplinary team, they play a crucial role in ensuring patients receive timely support, enabling them to recover safely at home and reducing the strain on emergency services. 

 

Alongside direct clinical activity the postholder will also have the opportunity to lead service development projects, coach and train internal and external staff and develop and promote the role of the urgent community response paramedic in Hounslow.

Qualifications

Experience
