Job overview

Marble Hill 2 is a busy acute medical ward that specialised in Endocrinology.  The ward is supported by members of the multidisciplinary team including Diabetic specialist nurses, dietitians, therapists and palliative care team.  Many of our patients have very complex needs and require careful discharge planning.

We are looking for a dynamic and forward thinking nurse with an interest in developing their leadership skills to support and manage a team of nurses to provide high quality patient care.

We welcome applications from nurses that have at least 2 years acute hospital experience at a band 5 level with a substantive background in caring for patients with Endocrinology conditions including Diabetes Mellitus, Addison’s disease, hyperthyroidism/hypothyroidism and can demonstrate knowledge of caring for patients with diabetic foot symptoms.  They must have the ability to care for acutely unwell patients and patients who are end of life.  They should be well organized have good administrative and interpersonal skills.

Candidates will need to have excellent communication skills and be able coordinate care of patients across the multidisciplinary team to ensure high standards of evidenced based care for every patient.  They should have experience in organising complex discharges and working with community partners including the palliative care team.

 

Main duties of the job

 ·         To anticipate the care needs of patients and actively manage the patient’s journey through the unit, including advanced life support and assessment.

 

·         To support, and work with the Ward Manager to provide effective unit management. Specifically this will include: staff management, stocks and  patients care delivery

 

·         To pro-actively participate in the planning, assessing, implementation and evaluation of patient care as part of the multi-disciplinary team from admission to discharge.

 

·         To deliver care in accordance with the NMC Code of Practice/Scope of Professional Practice and the Trust’s policies and protocols.

 

·         To act as an advocate for the patient and family at all times.

 

·         To ensure the safety of patients in accordance with local and national safeguarding procedures.

 

·         To participate in auditing effectiveness and incorporate finding into practice.

 

·         Be aware of own limitations and able to escalate issues appropriately

·      Ensure the service continually improves to meet the needs of patients.

·      Develop nursing staff to improve patient care by enhancing assessment and technical skills, and actively promoting advanced clinical skills and competencies.

·      Provide leadership that promotes effective discharge planning that incorporates the patient & carers view.

·      Provide expert advice on all aspects of patient care, through others where appropriate.

·      Monitor standards of cleanliness and the patient environment within the clinical area.

·      Ensure health and safety of staff and patients by complying with appropriate standards. These include radiological protection, decontamination, COSHH, Medical Devices, Infection Control and the safe administration of medicines standards.

·      Ensure staffing rotas incorporate effective use of resources, managing within financial envelope.

·      To achieve length of stay reductions through implementation of enhanced recovery programmes and effective discharge planning, including Fast track.

·      Deputise for the Ward Manager in their absence.

·      To make effective use of available resources and eliminate waste in order to contribute to the cost of effective delivery of services.

Education and qualifications

Essential criteria
  • NMC Registered Nurse (adult)
  • Nursing Degree
  • Qualification in mentoring and assessing
  • Relevant post registration qualification in Acute care (other than Diabetes)
  • Deteriorating patient course or equivalent
  • Diabetes course
Desirable criteria
  • End of life care /palliative care course
  • Relevant management qualification or willingness to undertake

Experience

Essential criteria
  • A minimum of 2 years band 5 RN experience working on an acute medical ward
  • Previous regular experience as shift leader
  • Previous experience caring for patients with Endocrine conditions including Diabetes
  • Previous experience using sliding scale insulin
  • Previous experience caring for patients who are end of life
  • Can demonstrate how to reduce risk within the workplace to ensure patients and staff are safe including clinical governance, safe management of medications, infection control and falls prevention
  • Previous experience dealing with complaints/difficult situations
  • Previous experience working with the MDT including specialist nurses
  • Previous experience looking after complex diabetic foot wounds
Desirable criteria
  • Previous experience managing junior staff and students
  • Previous experience in auditing

Skills and Knowledge

  • Have the skills and knowledge to manage an emergency situation
  • Demonstrates an understanding of the NMC Code of Professional Conduct
  • Awareness of current developments in own clinical specialty
  • Demonstrates evidence of developing innovative practice and of being proactive in managing change
  • Can demonstrate a good general knowledge of endocrinology conditions and treatment including procedures, scans and investigations
Desirable criteria
  • Have a good knowledge in discharge planning for patients with complex needs
