Job overview

An exciting opportunity has arisen for a full time assistant patient pathway coordinator to  join our Audiology, ENT and Oral Services Administration team.

Working as part of a multidisciplinary team you will support clinicians and management team with providing efficient and timely patient care

Main duties of the job

1. Process and log all referrals, including paper and Choose and Book, in line with Trust Access Policy and speciality SOP.

2. Schedule new and follow up appointments with the most appropriate clinician, ensuring that capacity is proactively and efficiently used and escalate to PPC where this cannot be achieved.

3. Ensure that any diagnostics and investigations that are required as part of the patient pathway have been requested, including those carried out at other Trusts.

11. To use the 18weeks referral to treatment (RTT) to manage all outpatient and elective patient journeys.

12. Assist in the validation of the RTT Patient Tracking List (PTL). Investigate and take the appropriate action, under direction, where pathways are incomplete to ensure that patients are receiving timely
treatment and that Trust Information is robust.

13. Support the identification and escalation of any issues to the PPC which compromises delivery of the 18 week RTT pathway, e.g. lack of capacity either in outpatients or theatres

19. Ensure that Trust agreed standard of service is delivered to patients requiring elective admission to speciality and escalate to PPC where this cannot be delivered

20. Ensure details of patients to be admitted are entered onto CRS, recording accurate information

21. Assist in the scheduling of elective admissions, contacting patients and offering them a choice of admission dates and agree with them the date of admission
