Role : Bank Band 5 Community Nurse

Do you want to make a difference? Then why not join the South Locality District Nursing Team in Tower Hamlets. We are looking for adult community registered nurses to cover a bank role within the Trust.
As a Bank Band 5 Community Nurse you will be providing high quality nursing care to patients who reside in the Borough of Tower Hamlets.
Main duties of the job
Our team is an established integrated service. This will give you the opportunity to work closely with our Physiotherapists, Occupational Therapists, Care Navigators, Mental Health Liaison Nurse and Psychologist. Our service is also supported by a team of experienced administrators. You will also have the opportunity to work closely with the local General Practices, Tower Hamlets Social Service team and other statutory and non-statutory services to meet the health and social needs of this local population. The integration and collaboration of all these disciplines and services allows all new admissions to our service to have a clinically robust assessment and treatment plan, with an emphasis to promote independent living, that is holistic, evidence based and in line with all current Best Practice Guidelines. Support and development for staff in our service is one of our main goals.

Desirable criteria
- DN Cert/Dip/Degree
- Non-Medical Prescriber V300
- Nurse Formulary prescriber V100
- Advanced history taking and physical assessment course
Experience
Essential criteria
- Good organizational Skills and ability to priories work and delegate effectively to the team
- Experience of working in a multi professional team and lead across organization to achieve high patient outcomes.
- Assess plan and implement a plan of care for individual patients
- Ability to liaise effectively with members of the Primary care team
- Ability to organize and deliver high standards of evidence based clinical nursing care
Knowledge and skils
Essential criteria
- Understanding and knowledge of the complex needs and long term conditions management.
- Knowledge of current professional issues facing nursing
- Knowledge of community nursing practice and current legislation.
- Knowledge of safeguarding issues and the protection of adults.
- Knowledge of information governance and the responsibility
