Job overview
An exciting opportunity has arisen for 12 month secondment for a Band 6 Frailty Nurse to join our existing team of Specialist Frailty Practitioners (nurses and therapists) in the delivery of the Royal London Hospital’s Frailty Service.
Our multidisciplinary Frailty Team delivers person centred care, frailty screening and comprehensive assessment and care planning for older adults with frailty admitted to the Royal London Hospital.
This post will be of potential interest to nurses who are working at Band 5 or 6 who have an interest and experience in working with older people. The successful candidate will have an understanding of frailty and the comprehensive geriatric assessment and a wish to progress and apply their expertise in a stimulating clinical environment.
We can offer strong support for development within our expert team, with help to develop your skills and knowledge as well as opportunities to take part in shaping the future of the frailty service and to deliver teaching and education on frailty for The Trust.
Main duties of the job
- The Frailty Nurse is a member of the Royal London Hospital Frailty Team, delivering multidisciplinary assessment and clinical management support for frail patients within the hospital, the emergency department and in the community.
- The Practitioner will carry out screening and specialist assessments of older people in the Emergency Department and throughout the hospital using a Comprehensive Geriatric Assessment model
- Plan and deliver interventions with the aim to facilitate safe and timely discharges and prevent avoidable admissions
- Promote excellent integrated, multidisciplinary and multi-agency working.
- To ensure that patients and their families are fully involved, informed and supported at all stages of the patient journey
- To establish clear lines of communication with primary healthcare teams and outside agencies in order to promote optimum service provision

- Post registration experience including a minimum of 1 year in acute medicine/care of the elderly.
- Experience in caring for vulnerable adults and understanding the issues relating to safeguarding with an in depth knowledge of caring for patients with dementia.
- Ability to demonstrate a clear understanding of MCA, DoLS and enhanced care.
Knowledge/Skills

- Is aware of how to identify and reduce risks with regards to health and wellbeing
