Job overview

As part of the London sickle cell transformation programme, we are seeking to appoint a highly motivated innovative and enthusiastic transition nurse.  The post is based at the Royal London hospital, but will require working jointly with hospital and community teams across the North East London (NEL) Network to enable a smooth transition to adult services.

Main duties of the job

The post holder will be an advocate for children and young people transitioning to adult care in Barts Health and hospitals that are within the East London and Essex networks. This will include 

•    Working closely with the multidisciplinary team to provide support,               advice and education for teenagers and young adults in inpatient and           outpatient setting up to the age of 25years
•    Working across paediatric and adult services within the trust.
•    Acting as a resource for teaching and educating patients and staff
•    Developing CYP strategic priorities

Person specification

Qualifications

Essential criteria
  • RN children’s /Adult
  • Preparation for mentorship course or equivalent
  • Evidence of continued professional development
Desirable criteria
  • Educated to degree level or working towards it

Experience

Essential criteria
  • • Minimum of 2 years’ experience at band 5 or above within with the speciality or a related clinical specialist environment
Desirable criteria
  • Haematology course

Skills

Essential criteria
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills
  • Organisational skills- able to incorporate priorities and work to deadlines when required.
  • The ability to provide nursing leadership and advice within the service and to lead and motivate staff.
  • The ability to apply flexibility to working practice to meet needs of service users and changes in service models
  • The ability and motivation to work creatively and positively with practitioners from other disciplines and agencies
  • The ability to work with others to plan, manage and implement service developments using service improvement methodologies
  • Ability to facilitate reflective practice and carry out task without supervision.
  • Skills in IT and maintaining accurate data.
