We currently have an exciting opportunity for a newly qualified Physiotherapist (Band 5) to join Central London Community Health Care NHS Trust (CLCH) community learning disabilities teams (CLDT).  For the right candidate, this is a real opportunity to shape patient care and put physiotherapy at the centre of LD community services. Main duties of the job

 You will work within a friendly, multidisciplinary team mainly in the clinic and domiciliary setting across the boroughs of Hammersmith and Fulham, Kensington and Chelsea, and Westminster providing the highest standard of care in the community, offering a community-based service to adults with learning disabilities from aged 18 years, as well as supporting transition from children’s services.  You will work closely with community services, primary care, care providers, equipment specialists and wheelchair services to provide physiotherapy assessment and management for an identified group of patients with complex needs.

You will be responsible for setting high standards of practice and be a flexible and adaptable team player. You must have excellent verbal, non-verbal and written communication skills, able to motivate and engage both care staff and clients.

Qualifications

Essential criteria
  • BSC or MSC degree in Physiotherapy
  • Current HCPC registration
  • NVQ Level 3 equivalent or higher in English literacy.
Desirable criteria
  • Member of CSP
  • Member of ACPPLD
  • A short course relating to the Postural Assessment of patients with spinal deformity.
  • Evidence of up-to-date CPD portfolio for the past 6 months.

Experience

Essential criteria
  • Community Learning Disability team
  • Paediatrics / Special Needs School
  • Neurology (inpatient / community)
  • Wheelchair Service
Desirable criteria
  • Experience working for a Learning Disability Team
  • Experience working in a paediatric special needs placement.
  • Experience in Rehabilitation and/or neurological rehabilitation.
  • Experience working with orthotics, wheelchairs and specialist seating.
  • Experience working as a Band 5 Physiotherapist greater than 6months
  • Experience working in the NHS
  • Experience working with adults with cerebral palsy.

Skills

Essential criteria
  • Knowledge of role of Allied Health Professionals and interdisciplinary team working
  • Ability to apply core physiotherapy skills across a range of clinical conditions and engage clients in goal setting.
  • Knowledge of outcome measures and evidence based practice associated with long-term conditions.
  • Ability to problem solve, prioritise and demonstrate clinical reasoning
  • Excellent verbal and non-verbal communication and timely report writing skills.
  • Competent IT skills – Microsoft Word, Power point, Excel, MS teams.
  • Knowledge and understanding of the role of Social Services and voluntary agencies
  • Able to provide pool-based therapy programmes.
  • Able to confidently and safely use public transport within the London area.
Desirable criteria
  • Skills in the assessment and treatment of adults with complex multipathologies including cerebral palsy.
  • Knowledge and understanding of complex medical conditions including cerebral palsy and other multi-pathologies.
  • Postural Assessment for Adults with CP
  • Knowledge of orthotics, wheelchairs, specialist seating
