Be responsible for the delivery and development of the Musculoskeletal Outpatient Physiotherapy Services within Central London Community Health Care Trust (CLCH).

 

KEY WORKING RELATIONSHIPS

 

·       Regularly liaise with local GP’s and other Health Professionals in relation to patient care and/or health promotion. To provide comprehensive discharge reports and clinical reports in a timely manner to referrers (such as GP) and patients and other relevant multi-disciplinary team members.

·       To consistently communicate effectively with patients and carers/family via clear verbal, non-verbal and written means to maximise rehabilitation potential. Use communication skills of negotiation, motivation, explanation and reassurance and working with interpreters when required.

·       Demonstrate compassion, empathy and sensitivity whilst providing an appropriate level of care according to the needs of the patient.

·       Working with other members of the team including junior and senior members of staff.

·       Be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care. Ensure a consistently high level of clinical care for the patients under your management and support your peers to do likewise.

·       Provide an equal and equitable service for all patients, including the harder to reach population groups.

·       Undertake a comprehensive assessment of patients, using investigative and analytical skills. Be aware of your professional limitations, seeking support as necessary through supervision, discussion with peers/more senior colleagues and team case discussions.

·       Use a variety of assessment and treatment techniques appropriate to the patient, in accordance with evidence based practice, national guidelines and trust and departmental policy (including manual handling and confidentiality).

·       Demonstrate compassion, empathy and sensitivity whilst providing an appropriate level of care for patients with acute and chronic, complex presentations (which may include victims of torture, mental health patients, etc.).

·       Triage referrals to ensure every patient is appropriate to the Musculoskeletal Outpatient Service in accordance with the triage guidelines.

  • Communicate complex condition related information effectively to patients and relatives to progress treatment programmes. This will include the use of advocates to enable families whose first language is not English to access the service.
  • Negotiate with others around case management in complex cases.
  • Ensure collaborative working. To communicate with those involved in the care of patients, including GPs, in a professional, concise, accurate and detailed manner via telephone, letter, email, or in person. This may include the sharing of sensitive information relating to patient progress, Physiotherapy input or other matters affecting clinical intervention.

Experience

  • Experience of working in an outpatient department
  • Experience of working within NHS environment
  • Experience of planning and delivering clinical supervision / mentoring to peers and less experienced physiotherapists
