Cancer Tracker (Cross site)

The post holder will be a member of the Cancer Services team that is specifically dedicated to supporting the streamlined pathway of cancer patients though the Trust.
The main purpose of the Cancer Tracker role is to support plan and journeys of patients on a Cancer Pathway. The post holder will need to support the trust in achieving performance against CWT guidance and ensure that all patient have clear next steps in place in line with CWT individual standards 28D, 31D and 62D.
The post holder will collect data from a variety of sources and update the Trust’s cancer database Somerset. The post holder will need excellent organisational and communication skills and a good attention for detail to be successful in the role.

- Fully track allocated tumour sites ensuring in line with 28D, 31D and 62D performance targets
- Attend and present at the weekly PTLs giving updates on patients and escalating where required
- Maintain an up to date PTL that is clear concise and relevant
- Ensure actions are raised on the FDP (Federation Data Platform) tool for next steps required and update tracking accordingly with updates and that all un- actioned actions on FDP are escalated appropriately
- Ensure that every patient with a cancer diagnosis is discussed at an MDT meeting and high risk where applicable.
- Develop and maintain effective working relationships with counterparts in other local Trusts.
- Work with the MDTs to ensure GPs receive confirmation of new cancer diagnoses within 24 hours of the patient being informed of their diagnosis.
- Assist with the monitoring of the generic inbox and ensure any patient requiring an upgrade is added to the Cancer Registry in line with internal process
- Ensure tracking is clear, concise and accurate and update any validations in relation to COSD (Cancer Outcomes and Service Data SET) and other required data monitored audit
- Enter accurate data into the Trust’s cancer database on a daily basis in order to facilitate the monthly upload of data to the Department of Health’s national Cancer Waiting Times Database.
