This is an opportunities for both experienced and newly qualified Registered Nurses (Adult) to join our Extended Primary Care Teams in Newham, East London as Band 5 Community Nurses
What we are looking for:
- Experienced qualified nurses (Registered Nurses Adult) who are looking to develop their skills and knowledge in community health practice.
- Newly qualified nurses who are looking for their first post as a qualified nurse.
- Nurses who offer a high level of person-centered nursing care.
- Nurses with excellent communication skills both verbal and non verbal.
- Nurses who can work as part of a Multidisciplinary team in order to enhance the quality of care provided to patients.
- Nurses who can work shifts (8am-4pm, 2pm -8pm) over 7 days a week (including Bank Holidays)

Detailed job description and main responsibilities
The post holder will be responsible for the planning, implementation, and evaluation of individualised nursing care in the community
The post holder will provide health education to enable patients to self-manage, and achieve optimum independence following deterioration or a period of ill health.
To participate in the training of students, the induction of new staff and others that have contact with our services.
The post holder will be required to support the senior nursing team in the maintenance of efficient and effective delivery of services.
Use a broad range of nursing knowledge, interventions and skills, to prevent, where possible and appropriate, an unnecessary admission to hospital.
Good communication links with patients and carers and joint working with other services across professional and organisational boundaries in order to provide a patient-centered, holistic and seamless service.

Qualifications
Essential criteria
- Registered General Nurse.
- Demonstrate evidence-based practice and up- to-date clinical skills.
Desirable criteria
- Evidence of post-basic professional development
- Relevant teaching/mentorship qualification or commitment to undertake.
Experience
Essential criteria
- Evidence of and commitment to multi- professional working.
Knowledge
Essential criteria
- Literate in IT/Computer Skills
Other
Essential criteria
- Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.
Desirable criteria
- Current driving license.
