Job overview

This is an opportunity to join a well-established mental health care for older persons community mental health team (MHCOP CMHT) in Newham. The post holder will work alongside another substantive full-time consultant. The CMHT provides comprehensive and multidisciplinary community mental health care to frail and older persons. The diagnostic memory clinic is co-located, closely aligned to the CMHT but has separate consultant leadership.

Main duties of the job

To provide comprehensive assessment, diagnosis and medical management of patients, by delivering the
following interventions:
 Comprehensive psychiatric assessments, with development of care plans, taking into account
patients’ capabilities, strengths and resilience
 Mental state examination
 Clinical diagnosis
 Initiation and reviews of pharmacological treatments
 To lead multi-disciplinary assessment, formulation and management of individual’s risk.
 Physical health assessments
 Referrals to other services (medical referrals, diagnostic tests, forensic services)
 Liaison with GP, community and forensic services.
 Reviews of clinical progress and care plans
 Assessment of readiness for transfer to less restrictive setting and discharge planning.
 Safeguarding planning processes
 Consultation and liaison with families and carers.
 Liaison with the criminal justice system, police, Courts and probation when required for the care
and management of patients.

Qualifications

Essential criteria
  • Fully registered with the GMC with a licence to practise at the time of appointment
  • Included on the GMC Specialist Register OR within six months (older adult or dual older adult/adult)
  • Approved under S12 OR able to achieve within 3 months of appointment
  • MRCPsych or equivalent
  • Approved clinician status OR able to achieve within 3 months of appointment
Desirable criteria
  • Qualification or higher degree in medical education, clinical research or management.

Skills/Abilities

Essential criteria
  • Ability to work as part of a multi-disciplinary team
  • Excellent clinical skills using bio-psycho-social perspective and wide medical knowledge
  • Ability to manage clinical complexity and uncertainty
  • Ability and commitment to work with service users in service development and delivery
Desirable criteria
  • Formal leadership/management training

Knowledge

Essential criteria
  • Excellent knowledge of operation of acute adult and older adult mental health service
  • An understanding of relevant Government guidelines, targets and legislation
  • Understanding of social care structures relevant to older adults
  • Excellent knowledge of mental health and capacity law, in particular application of the MCA 2005

Clinical experinece

Essential criteria
  • Excellent experience of operation of older adult mental health service
  • Assessment and treatment of a wide range of mental health and psychological disorders with evidence-based interventions
  • Assessment of cognitive impairment and diagnosis of dementia and delirium
Desirable criteria
  • Wide range of specialist and sub-specialist experience relevant to post within NHS or comparable service
  • Experience of contributing and/or leading on service planning, development and management
