Senior Occupational Therapist

The Extended Primary Care Services enable the provision of multidisciplinary care and support to vulnerable adults, with the most complex medical and social needs, in a range of community settings.

Occupational therapists are an integral part of the MDT.  You will be joining a team of OTs, physiotherapists, trusted assessors, OT technicians and rehabilitation support workers in assessing patients who have been discharged from hospital and/or recovering from an illness in order to support them in their rehabilitation.

Time                    :             09:00 am – 16:00 pm (Come prepared to be interviewed on the day)

Morning registration: 9:00 AM -10:00 AM (We wont accept candidates for the morning session after this time)

Morning Interview    : 10 am - 12:30 pm

Afternoon registration: 12:30 - 13:15 pm (We wont accept candidates for the afternoon session after this time)

Afternoon Interview   : 13:15 - 16:00 pm

The event is an all day event. Candidates need to register for either the morning session / the afternoon session. Failure to register during the registration periods will mean inability to take part in interview. Please submit your application to be considered.

 

 

 

Main duties of the job

The main aim of Extended Primary Care Services is to enhance independence following a hospital admission and/or a deterioration in physical health,  supporting patients in their own homes. Occupational Therapists  work as part of the multidisciplinary team, providing specialist client-centred occupational therapy interventions, to enable clients to maximise their independence in daily activities in their own homes.

Person specification

Education

Essential criteria
  • OT with a valid HCPC registration
  • Advanced Physical Assessment Skills or willingness to undertake course
  • Evidence of continuing professional education and training at degree level
  • Management and leadership skills
  • Experience of integrated working
  • Teaching and mentorship qualification

Experience

Essential criteria
  • Minimum of 18 month clinical ,postregistration physiotherapy practice, which should include working with a range of clients who require physiotherapy in therapeutic setting in the community
  • Evidence of professional development
  • Evidence of recent clinical practice
  • Knowledge of HCPC Code of Conduct
  • Professional Conduct and Scope of Practice
  • Evidence of leadership, change management and motivational skills
  • Knowledge and understanding of current health and social care policy, i.e. National Service Frameworks
  • Knowledge of principles of Clinical Governance

Knowledge

Essential criteria
  • Knowledge of theory and practice of physiotherapy with the clients with physical ,cognitive and psycho-social needs
  • Knowledge of legislation affecting our work as physiotherapists
  • Evidence of sound knowledge of the role of other disciplines and agencies involved
  • Completing an initial assessment and formulating a care plan
  • Ability to effectively manage the caseload, prioritise and delegate work
  • Ability to work within a team of community nurses and ensure Trust policies and procedures are followed
  • Ability to organise own workload, work under pressure and forward plan team activities
  • Ability to communicate information effectively to the Primary Health Care Team and other professionals/agencies
  • Evidence of good communication skills both written and verbal
