An exciting opportunity has arisen to join our vibrant Adult Community Nursing Service. Homerton Healthcare NHS Foundation Trust is an integrated community and acute trust providing high quality service to residents of City and Hackney borough.

We are seeking for enthusiastic Band 5s to fill full time (37.5) posts. Adult community Nursing Service is divided in 8 neighbourhood teams. The successful candidate will be asked to work within one of the neighbourhood team.

Main duties of the job

  • The post holder will practice within a multidisciplinary team and provide care to patients within their own home and community settings
  • Assessing, planning, implementing and evaluating their care needs under the supervision of Clinical Nurse Specialists, Practitioners and District Nurses.
  • The successful candidate will be responsible for delivering a high standard of holistic nursing care whilst promoting and maintaining health education and independence.
  • The post holder will take responsibility for their own actions when delivering clinical care; maintaining competency and skill in line with role requirement.
