An exciting opportunity has arisen to join a new, innovative and dynamic team within our expanding Frailty Virtual Ward service. We are looking for experienced and motivated senior nurse (Band 7 or current 8a) who is passionate about service development and providing high standard of patient centred care.
You will be working closely within a Multi-Disciplinary Team of staff including Doctors, senior nurse practitioners, Occupational Therapists, Physiotherapists and HCA’s, all of whom are committed to maintaining independence, promoting self-care and assisting patients in their usual place of residence. This role will require strong leadership and visible, credible senior nursing presence accessible to staff, patients and their families. At times this post may require you to work autonomously, when working across the Hackney borough into patients own homes and in an inpatient hospital setting.
Main duties of the job
Providing specialised advanced high-quality care and support for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital/ other healthcare settings under direct or indirect supervision of senior medical colleagues.
• This role will be at an advanced clinical level in the direct delivery of high quality care making critical clinical decisions based on advanced clinical skills assessment within the community setting.
• The post holder will provide clinical leadership and support within the Frailty Virtual Ward nursing/AHP team in developing the service to improve the patient experience and clinical outcomes while remaining in their own homes.
• Responsible for ensuring the operational continuity, financial management and strategic development of the Frailty Virtual Ward Service.
• To communicate to a high level with all key stakeholders, for example GP’s, social services, acute hospitals, care providers and voluntary services.
• To maintain overview of the current team operations and support the service management team to ensure effective service delivery to meet KPIs, Trust objectives, local and national standards and progress teams in line with emerging initiatives.
