The post holder will be responsible for leading and managing the Advanced Kidney Care Nurse Specialsit Team. You will independently manage your own patient caseload, including all aspects of clinical care in relation to chronic kidney disease.
You will be responsible for leading a specialised service, esuring that patients with progressive chronic kidney disease receive the highest standard of care in preparation for renal replacement therapy or supportive care.
You will be a resource for patients and the clinical team being an expert in the specialty.
Prif ddyletswyddau'r swydd
- Provision of specialist, high quality, expert nursing care and support for patients (and their carers) with progressive chronic kidney disease in liaison with the multidisciplinary team(s) and other health agencies.
- To act as a resource for patients and the clinical team by the provision of comprehensive advice, information, teaching and training with regard to chronic kidney disease both within ICHT and externally.
- To have active involvement in the development of the service locally and nationally implementing and adhering to guidelines such as Renal Medicine GIRFT and NICE NG107.
- To undertake, where appropriate, nursing research with published outcomes, and updating own knowledge to promote excellence in clinical practice, and the setting of quality standards within speciality.
And contribute in audit processes. Formulate an annual report/clinical audit of own CNS service and outcomes.

Education/ qualifications

- NMC Registration on relevant part of register
- Minimum of first degree and post registration courses (CPD) relevant to clinical speciality
- Evidence of continuing personal and professional development
- Management or leadership qualification or equivalent experience.
- Masters qualification or relevant masters level experience
- Willingness to undertake non-medical prescribing course if not already in possession of this

- Has attained or is working towards Master’s Degree
- Counselling/Communication course/certification
- Proven track record as specialist with extensive professional portfolio of publication, presentation, research and clinical audit
- Non-medical prescribing
Experience

- Previous experience as an advanced kidney care nurse specialist
- Significant post registration experience in the specialty
- Experience of teaching in a variety of settings and variety of audiences
- Experience of staff management
- Experience in change management
Skills

- Proven skills in patient assessment within the advanced kidney care setting.
- Evidence of applying current research to practice.
- Basic IT skills
- Experience in conducting audits.
