Job overview

Op RESTORE is an NHS service hosted by Imperial College Healthcare NHS Trust that supports individuals who have served in, or are leaving, the UK Armed Forces and have continuing, physical health injuries and related medical problems attributed to their time in the Armed Forces. 

This is an opportunity for a motivated, driven and enthusiastic individual with a passion for veteran’s healthcare or with experience of working in the Armed Forces to help the service support the wellbeing of veterans as they proceed down the correct physical healthcare pathway for their service-related injuries or illness.   

This is a non-clinical role and successful candidates will have a strong welfare/wellbeing background and have experience of delivering welfare/wellbeing support to patients as well as a good admin background and be comfortable working on their own.  They will be confident to liaise with military charities, NHS providers and other veteran programmes to support the wellbeing of veterans referred into the service.  All patient interaction is via the telephone and this will include liaising with primary care, social prescribers and other support agencies. 

Please note this is a homeworking post – the expectation is that the post holder will be based at their home with occasional travel into London or other parts of England.

Main duties of the job

  • Op RESTORE is an NHS service that supports individuals who have served in, or are leaving, the UK Armed Forces and have continuing, physical health injuries and related medical problems attributed to their time in the Armed Forces. The service supports the referrals for veterans received by GPs and provide oversight and direction to the onward clinical pathways for veterans.
  • To  support veterans accessing the service by providing a wellbeing call to determine the holistic support that can be provided by the service and the wider Armed Forces community.  The Pathway Navigator will be completing assessment checklists on ADLs and  supporting the onward navigation of statutory and non-statutory services in support of the service. This role will require liaison between charity providers, primary care, other Armed Forces programmes such as VCHA Veteran Aware accreditation and RCGP Veteran Friendly accreditation and occasional attendance at external meeting presenting on OpRESTORE.
