Our hospitals and community sites provide a wide range of inpatient and outpatient services, as well as emergency and planned care. The Trust is rated as “good” or “outstanding” in over three-quarters of the services inspected by the Care Quality Commission.

Job overview
The post holder will assist the Hepatology team in the maintenance and ongoing management of HCC (Hepatocellular carcinoma) Surveillance Patient database.
To work with the Service Manager, the administrative team, and clinicians to support the pathway of Hepatology patients across both University Hospital Lewisham and Queen Elizabeth Hospital.
Responsible for reviewing and validating HCC surveillance data to support timely surveillance and follow up.
Track designated HCC Surveillance Patient throughout their pathway, escalating issues of concerns promptly and arranging next steps where appropriate.
To work as a team providing cross cover for the other members of the Hepatology administrative team across the Trust.
Main duties of the job
The post holder will work closely with the Service Manager, the administrative team, and clinicians to support the HCC Surveillance Patient pathway.
To maintain and manage the database of HCC Surveillance Patients
To work with Service Management and Business Intelligence team to review and further develop an automated HCC Surveillance Patient database as required.
To liaise with the Outpatient Booking Appointments Office to ensure all patients have a booked appointment, investigation and procedure.
Record details of patients referred via other routes other than General
Practitioners and add to the database. Report to the Service Management teams any deviations from process.
To administer entry into the database according to guidelines and monitoring milestones.
Undertake validations and corrections to the HCC Surveillance Patient pathways ensuring the pathway is kept up to date.
Ensure data is entered in a timely manner, checking accuracy and detail at all times.
Negotiate with all clinical and administrative personnel within the Trust to establish good working relationships in order to improve the coordination of the patient journey.

• Participate in weekly tracking meetings with Service Management, highlighting any issues with patient pathways which may cause them to not follow HCC Surveillance guidelines.
• Maintain patient tracking logs by updating appointments, clinic outcomes, results, updates and steps in patient journeys on a daily basis.
• Monitor patients to ensure six monthly HCC surveillance is maintained, proactively identifying overdue investigations and escalating concerns where appropriate.
• Liaise with patients following missed appointments, where appropriate, to support rebooking and ensure continuity of the surveillance pathway.
• To communicate and liaise with clinicians and their teams within Lewisham and Greenwich Trust in order to get information for the list of patients that require HCC Surveillance. This will be achieved through face to face contact, telephone, or email communication. Trust Data Protection Policy must be followed at all times.
• To develop strong working links with relevant members of the Trust, wider stakeholders and a number of external staff from across the network.
• To ensure any patient notes for investigation or audit are ‘tracked’ accordingly.
• To identify any missing information that cannot be found after thorough investigation and highlight this promptly to the Directorate service management teams.
• To highlight individual patient data to appropriate clinical teams within the Trust.
• To provide clerical support to any Hepatology project as required.
• To be involved in patient pathway investigations with the aim of identifying blockages, duplicates and subsequent areas where the patient journey can be streamlined.
• To provide accurate data management on any outpatient waiting lists.
• To liaise with the medical secretariat and clinical teams to identify patient non- availability both medical and non-medical.
• To support the Hepatology team and cross cover the other members of the team during periods of annual leave, absence, or need.

Essential criteria
- Educated to GCSE including English and Maths or equivalent
- Educated to level of diploma or equivalent level of experience requiring ongoing on-the-job training
- Prior secretarial/administrator experience
- NVQ III or equivalent
