Our hospitals and community sites provide a wide range of inpatient and outpatient services, as well as emergency and planned care. The Trust is rated as “good” or “outstanding” in over three-quarters of the services inspected by the Care Quality Commission.

Job overview

As a  Learning Disability Well-Being Practitioner and a member of Well -Being Practitioners Team  you will play a  vital role  in ensuring patients with learning disabilities receive safe compassionate and person- centred care during their hospital stay. You will also support individuals with a to access acute healthcare services by delivering  practical and emotional support, including one-to-one assistance during hospital appointments and admissions,  promote and facilitate the use of reasonable adjustments and accessible communication tools (e.g., Easy Read materials, Hospital Passports) to reduce health inequalities and improve patient experience under the supervision of the Learning Disability Nurse.

 

This role requires empathy, adaptability and teamwork. You'll need strong communication skills, the ability to stay calm under pressure, and a commitment to improving patient experience.

Main duties of the job

The post holder will be expected to work with patients admitted to the Emergency Department and Inpatient wards. As a member of the multidisciplinary team, delivering a high standard of evidence-based care and treatment in order to maximise each patient individual potential.

Deliver person-centred care to patients with learning disabilities and autistic people during their hospital stay.

Support registered nurses and the multidisciplinary team with clinical and non-clinical tasks.

Engage patients in activities and provide emotional support to them and their families.

Complete documentation such as Learning Disability and Autism hospital passports.

Assist with audits, surveys, and staff training across the Trust.

Communicate effectively with patients, carers, and staff, adapting your approach to individual needs.

Detailed job description and main responsibilities

  • To assist with the delivery of health education, data collection for outcome monitoring, and behavioural support interventions under the direction of the Learning Disability Nurse Specialist and other clinical professionals.  To contribute to training delivery, service promotion, and administrative tasks to support the operational effectiveness of the Learning Disability Service across the hospital.  

  • Deliver therapeutic interventions that will reduce agitation and distress and promote recovery to individualsTo identify factors that increase and decrease agitation/distress for the patient. To always deliver care according to the principles of least restriction. Carrying out assigned tasks under the supervision of the Clinical Nurse Specialist delivering a high standard of evidence-based care and treatment in order to maximise each patients’ individual potential.  

  • To use initiative to undertake some aspects of care provision without direct supervision. Ensuring the safety and well- being of services users and always working in a manner that promotes dignity and human rights through the adoption of holistic, person-centred care principles.  

  • Have a good knowledge of the inequalities that someone with a learning disability may face and how to ensure equitable access to health care services within the legal frameworks.  

  • Deliver care that has already been planned by a clinician or a Registered Nurse and will be skilled in taking appropriate action to ensure that outcomes are achieved.   

  • Be competent, confident and adaptable and will require minimal supervision from the registered professionals.   

  • Contribute to the efficient running of the service and ensure patients and their families are seen in a timely manner. 

Person specification

Qualifications /Education and Training

Essential criteria
  • Good level of general education including GCSE English and Maths
  • NVQ level 4
  • Foundation Degree or Degree in a healthcare related subject
Desirable criteria
  • Nursing Associate on NMC Register

Previous Experience

Essential criteria
  • Minimum 6 months experience in acute hospital setting or clinic
  • Previous experience of working with patients with mental health conditions.
  • Previous experience of working with patients that present a risk to themselves or others.
  • Previous experience of working as a healthcare assistant in a hospital or registered care setting
  • Experience of taking clinical observations Customer care experience.
  • Ability to document legibly and use electronic patient system when needed
  • Demonstrates awareness of recourse management
Desirable criteria
  • Experience of therapeutic de-escalation skills.
  • Experience of working within a team of mental health professionals.
  • Previous experience of working in a health or social care setting, either statutory or non-statutory service
  • Experience of delivering psychology interventions.

  • Knowledge of mental health conditions.
  • Knowledge of therapeutic interventions delivered to: • Reduce the risk of aggression·, Reduce distress caused by mental health symptoms such as hallucinations and mania.• Reduce the risk of self-harm · • Reduce anxiety· • Reduce self-neglect.
  • Able to interact well with multidisciplinary team and Patents/relatives.
  • Understanding of the requirements of patient confidentiality.
  • Knowledge of mental health admission and discharge pathways Punctual and reliable.

  • An awareness of the principles of social inclusion and recovery.
  • Knowledge of the Mental Health Act / Mental Capacity Act.
