An exciting opportunity has arisen to join the National Hospital for Neurology & Neurosurgery as a Motor Neurone Disease (MND) Care Centre Coordinator (Band 7). 

This is a fixed term post for 12 months covering maternity leave, supported by a Motor Neurone Disease Association (MNDA) grant.

Working within a nationally recognised MND Care Centre, you will contribute to the coordinated, proactive and multidisciplinary care model recommended in NICE NG42. The role focuses on collaboration, working closely with the MND Clinical Nurse Specialist, Advanced Nurse Practitioner, Consultant Nurse, Neurologists and the wider MDT to ensure seamless, person centred support from diagnosis through to ongoing management.

You must have an understanding of the needs of patients with a life limiting illness; possess excellent communication skills for advising and supporting patients, relatives and other health professionals along with a willingness to learn about Motor Neurone Disease (MND)

You will join a supportive, forward thinking team dedicated to improving outcomes and experiences for people living with MND.

This is a full time, Monday to Friday role with no weekend or bank holiday working an opportunity to make a meaningful difference within a collaborative, specialist environment.

Main duties of the job

The Band 7 MND Care Centre Coordinator will hold an appropriate professional qualification (RN or AHP) and work collaboratively within the MND team and wider multidisciplinary network to deliver coordinated, proactive and person centred care in line with NICE NG42 guidance.
The postholder will:
•    Practice autonomously while working in partnership with the MND Clinical Nurse Specialist, Advanced Nurse Practitioner, Consultant Nurse, Neurologists and community based MDTs.
•    Coordinate care for people living with MND by attending borough specific MDT meetings, facilitating referrals and maintaining accurate records on the MND register.
•    Contribute to the development and review of protocols, pathways and organisational guidelines, ensuring compliance with NICE NG42 and the National Service Framework for Long Term Conditions.
•    Act as a clinical resource for colleagues and students, promoting evidence based practice and supporting education within the specialist area.
•    Participate in audit, service evaluation and quality improvement initiatives to enhance patient experience and service delivery.
•    Continue to develop specialist clinical knowledge and apply research and critical analysis to practice.
•    Build strong clinical networks and maintain effective communication across acute, community and voluntary sectors.
