Are you a passionate, energetic and an experienced Community Children’s Nurse?
An exciting opportunity has become available for a full time children's nurse to work within the Bexley and Greenwich Community Children’s Nursing and integrated Hospital at Home Team.
The post holder will be a children’s nurse with extensive knowledge and experience of working within the community, thus being an exceptional role model and able to demonstrate evidence of up to date academic development post registration.
The Community Children’s Nursing team is an established, motivated team and a highly valued service within the boroughs of Bexley and Greenwich; having well embedded and close links with the local acute and tertiary hospitals.
The team is community based and provides a seven day service from 08.00 to 20:30, there is also an on-call provision for our Continuing Care service and end of life, of which you will be required to participate.
The post holder will be responsible for participating within the development and implementation of practice within the CCNT, ensuring evidence based practice is being delivered. The post holder will be required to work closely with the Integrated Children’s Nursing Team and Children’s Continuing Care Team.

To provide a high standard of evidenced based nursing care for children and young people with the following conditions: acute and long term conditions, children with disabilities and complex conditions including those requiring palliative and end of life care. To provide clear visible leadership and professional clinical expertise to the Band 6 Children’s Community Nurses in the whole team to ensure every aspect of the child and family's experience is delivered in a safe, efficient and evidence based manner.
To work as part of a multidisciplinary team with Paediatricians and GPs providing support, advice and care co-ordination for children, young people and families in the management of their holistic needs. The post holder will be involved in scoping and implementing acute pathways between the acute hospital and community and between GPs and the community.
To be a resource offering education and support to families, other community services schools and voluntary organisations to ensure continuity and equity of service to all patients.
To manage and supervise the work of Band 6 Children’s Community Nurses in the and Hospital at Home service, allocating work and overseeing case management.
To cover for colleagues/caseload as required.

- Evidence of successful completion of relevant, specialist courses equivalent to Master’s degree
