We are looking for an experienced, dynamic, and compassionate Lead Pharmacist to join our supportive and forward-thinking Medicines Management Team in Ealing. This exciting leadership role offers the opportunity to drive change, improve patient outcomes, and support integrated care across a wide range of community services. You will lead medicines management for our Rapid Response Virtual Ward, a key service that helps avoid hospital admissions and enables early discharges. Additionally, you’ll provide expert pharmacy support across Ealing Community Health Services, including a wide range of specialties   e.g musculoskeletal (MSK) care, diabetes services, district nursing, special schools, children with learning disabilities, speech and language therapy (SALT), physiotherapy, dietetics, podiatry, tissue viability nursing (TVN), and more.

What You’ll Be Doing

  • Act as the lead pharmacist for the Rapid Response Virtual Ward, advising on all medicines-related issues and designing medication optimisation pathways.
  • Deliver expert medicines management and information to multidisciplinary community teams.
  • Ensure safe, clinically effective, and cost-efficient use of medicines across services.
  • Provide prescribing advice and support to clinicians, patients, and carers.
  • Lead on audits, clinical governance, training, and reporting of medication-related activities.
  • Develop, monitor, and implement medicines-related policies and guidelines.
  • Collaborate with service leads and the Head of Pharmacy to deliver the annual work plan.
  • Participate in the weekend and out-of-hours on-call pharmacy rota (additional pay applies).

Essential criteria
  • Masters Degree in Pharmacy or equivalent
  • Registered member of the General Pharmaceutical Council
Desirable criteria
  • Non-Medical Prescriber
  • Clinical Certificate or equivalent experience
  • Clinical Diploma or equivalent experience

Experience

Essential criteria
  • With relevant and significant pharmacy experience relevant to the post
  • Experience in providing a medicines management service or equivalent
Desirable criteria
  • Experience in integrated care (or equivalent) environment Undertaking clinical medication reviews in older people and improving adherence
  • Delivering direct pharmaceutical care to patients with long term condition
  • Management experience Management skills e.g. recruitment/ appraisal /training/ leadership/ motivation/ sickness absence
