An exciting opportunity has arisen within our proactive Critical Care Outreach Team for a 12 months fixed term contract. We are seeking an enthusiastic experienced and highly motivated critical care nurse who will work with the team to support us in the delivery of a high-quality 24-hour service across both Chelsea and Sutton sites. You will work collaboratively to provide acute oncology care to at risk or deteriorating cancer patients. You will carry out advanced assessment and care of deteriorating patients leading to early intervention and treatment. Excellent communication skills and the ability to work autonomously is essential. You will be part of the Resuscitation team/Hospital@Night and would be required as an ALS provider to lead rescue interventions and facilitate transfers to and from critical care. You will work a mixture of days and nights as well as weekends within the team.
You will be involved in the service development, improvement and participate actively in audit and research. You would need to demonstrate leadership and managerial expertise as a Band 7 Charge Nurse/Sister in Critical Care Outreach.
Main duties of the job
- To provide a 24-hour Critical Care Outreach (CCO) service across the Chelsea and Sutton site.
- To be an active member of the hospital at night (H@N) team.
- To identify patients who are acutely deteriorating and are for active treatment.
- To work in collaboration with the Critical Care Unit team (CCU), Medical/Surgical teams and the Clinical Site Practitioner (CSP) to assess and treat the acutely deteriorating patient.
- To follow up all patients discharged from CCU/Step-up to ensure patients do not deteriorate clinically on step down from the unit.
- To be an active member of the hospital resuscitation team.
- To provide support and education for nursing and medical staff in relation to the acutely deteriorating patient.
- To safely escort patients on intra/inter hospital transfers when required.

- To work in collaboration with the CCU, Medical/Surgical teams and the CSP to assess and treat the acutely deteriorating patient.
- To perform individual clinical assessment of patients condition utilising advanced clinical assessment skills.
- To utilise advanced nursing skills such as arterial puncture, venepuncture and cannulation to further assess and treat the acutely deteriorating patient.
- To prescribe specific medications when required either as a patient group directive or as a non-medical prescriber.
- To be skilled in the accurate making up of and administration of complex intravenous medications.
- To lead/advise on proactive interventions for the acutely deteriorating cancer patient.
- To initiate and coordinate the care of Level 2 patients being cared for in the inpatient or outpatient environment, or those stepped down from CCU.

Education / Qualifications
Essential criteria
- Professional Qualification (First Level registration NMC Registered Adult Nurse or HCPC) with relevant experience in a critical/acute care environment
- UK recognised Bachelor’s degree or studying at degree level supplemented by specialist training/experience
- Teaching/Mentoring qualification and/or experience
- Recognised acute care Qualification (e.g Post registration ICU course)
- ALS provider Certificate
- ILS provider Certificate
- Venepuncture/Cannulation
Desirable criteria
- Non-Medical prescriber
- Cancer nursing qualification
Experience
Essential criteria
- Significant and recent experience working in an acute setting and managing a critically ill patient (e.g CCU, A&E)
- Recent clinical experience at AfC Band 6 or above
- Evidence of good leadership and interpersonal skills
- Ability to work autonomously
- Ability to influence, support and develop staff
- Ability to co-ordinate patient care and liaise with multi-disciplinary team members
- Ability to work collaboratively with CCOT and CCU leads
- Evidence of managerial experience

- Computer literate in Microsoft outlook, office, word, excel and PowerPoint
