Job overview
An exciting opportunity has arisen for an experienced and motivated NMC registered nurse to join the established and highly regarded Richmond Community Dementia Care Team which is part of Kingston and Richmond NHS Foundation Trust (KRFT). KRFT provides high quality, holistic community health services for people living in the London borough of Richmond Upon Thames.
You would be based in Teddington and would form a key part of the community service. You would be managing a caseload of people living with dementia and supporting their informal carers. This includes carrying out high level clinical assessments for those living with dementia and their carers to deliver evidence-based interventions and advice to enable them to live as independently as possible within the community.
Enthusiasm, energy and a passion for delivering a high-quality service is essential. Previous clinical experience working with people with dementia and supporting their carers/family is also essential. Ideally you would also have formal knowledge of dementia acquired through clinical post-graduate training/education. Another aspect of this role is to support the service to deliver dementia awareness training for staff.

Main duties of the job
The postholder will provide:
- Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs.
- Facilitation of education and training for staff in the delivery of evidence-based dementia care.
- Clinical leadership with policy development and quality improvements in relation to dementia care.
- Consultancy to other health and social care professionals regarding dementia.

Qualifications
Essential criteria
- Registered Nurse (RMN/RNMH/RGN/)
Desirable criteria
- Evidence of continued professional development including post-graduate training/ education in dementia/older people
Experience
Essential criteria
- Significant post-registration clinical experience of working with people with dementia and their carers/supporters in different settings, including community/primary care.
- Experience of completing holistic assessments and providing bio-psychosocial interventions.
- Experience of working as an autonomous practitioner
Desirable criteria
- Experience of working within or alongside primary care teams and Collaborative multi-agency working
Skills/Abilities
Essential criteria
- Demonstrate the ability to treat families affected by dementia with respect and dignity, adopting a culturally sensitive approach that considers the needs of the whole person.
Desirable criteria
- Enhanced communication skills including experience of difficult conversations and a willingness to discuss and negotiate issues and ideas with the appropriate team / individual
Knowledge
Desirable criteria
- Knowledge of Information Technology to improve the delivery of care including SystmOne, Outlook, Excel and Word
