Diabetes Specialist Nurse

Job overview

  •  Due to resignation as a result of relocation,   an exciting opportunity has arisen for a Permanent post for band 6 diabetes specialist nurse 
  • To work as a member of the multidisciplinary diabetes care team in hospital.
  • Provide education, advice and support to inpatients and outpatients with diabetes and their carers, under the supervision of the Diabetes Specialist Nurses. 
  • Work collaboratively with multidisciplinary team to promote integrated care for inpatients within the diabetes care. 

Main duties of the job

  • To provide a high standard of diabetes care. To act in a consultative role to patients, relatives, and health professionals on the management of diabetes mellitus.
  • The post holder will utilise skills to assess and identify inpatient needs and implement delivery of appropriate individualised care for people with diabetes.
  • The post holder will take daily responsibility for managing a defined caseload, with the support of existing nursing and medical staff.
  • Specialist holistic advice and education will be provided for in patients and their carers, referring on as appropriate for follow up care on discharge.

  • Must be an RN/RGN with first level degree

  • Relevant Teaching and Mentoring course

  • At least 3 years’ post registeration experience experience
  • Previous experience caring

  • Excellent communication skills and abel to negoitate
  • Computer Literate
  • Sound knowledge of the care needs for people with diabetes

  • Knowledge of the relevant policy relating to diabetes services
