Band 8a Lead Nurse for Guided Care Matrons

To work in collaboration with partner organisations providing strong operational nurse leadership to the Guided Care Matrons and team members as part of the Care Connection Team (CCT) to   ensure the delivery of high standards of patient centred nursing care for the residents of Hillingdon.  

Main duties of the job

An opportunity has arisen to co-lead the  Care Connection Teams (CCT) . Alongside one other lead, you will be responsible for the leadership, management and development of the Guided Care Matrons and otehr members of staff within the Care Connection Teams (CCT).

The Care Connection Teams are part of the Hillingdon Health Care Partnership (HHCP)  . They proactively support people over 18 years old that have long term conditions and are at risk of hospital admission. The Care Connection Teams are aligned to neighbourhoods, based in GP practices. They are a team comprised of Guided Care matrons, GP’s, Senior Administrator Coordinators, Health Care Support Workers, Pharmacists, Health and Well-being Officers and Mental Health Nurses.

Interview’s will consist of Q&A, and a stakeholders panel.

Detailed job description and main responsibilities

You will be responsible for ensuring all aspects of the day to day operational delivery of the Care Connection Team. You will be expected to be visible, providing clinical support when necessary. You will be working alongside senior management, commissioners, the CCG and multi-agency providers, families and carers.

You will be expected to work closely with managers from partner organisations to line manage the Guided Care Matrons and team members to meet CCT targets. You will need to be forward thinking and able to provide integrated leadership. This role requires excellent leadership, organisational, time management and communication skills. This role provides an opportunity to play an integral role in shaping community services for the future.

The successful candidate's main duties will be:

  • Act as a role model- demonstrating CNWL core values
  • Day to day operational management of the Guided Care Matron and other team members.
  • Act as a point of escalation
  • Manage the service to consistently meet targets and KPI's
  • Over see the service budgets
  • Manage quality Governance to provide a safe effective service
  • Promote innovation
  • Engage with Patients, relatives and carers to ensure services meet their needs
  • Support the Adult Services Manager

Essential criteria

  • Advanced patient assessment skills

  • Evidence of ability to communicate both verbally and in writing at all levels in a multidisciplinary forum and with patients and carers

Experience and Knowledge

Essential criteria
  • Extensive post-registration clinical experience at a senior level
  • Experience of working in a community environment providing care to people with long-term conditions
  • Experience in management, quality improvement, audit and evaluating clinical nursing services
  • Experience of providing Clinical supervision
  • Experience of team/staff management including PDP & appraisal
  • Experience in team management including performance management
  • Experience of implementing NMC / NICE Guidelines & Essence of Care Benchmarks
  • Experience of working collaboratively with a variety of agencies and professionals across organisational boundaries
  • Evidence of innovation in practice
  • Knowledge across a range of long term conditions, including stroke and rehabilitation
  • Knowledge of current NHS organisational structures and policy initiatives
  • Knowledge of implementing and monitoring service outcome measures
  • Knowledge of resource management
  • Knowledge of clinical governance and quality improvement processes, risk assessment and personal accountability
Desirable criteria
  • Experince of working in acute and or speicailist nursing settings
  • Other clinical leadership/ improvement leader experience
  • Knowledge of Safeguarding, MCA, DOLS legislation

Qualifications

Essential criteria
  • Registered Nurse – current NMC registration
  • Evidence of ongoing and recent post-registration training and education.
  • Post-registration qualification in district nursing or other relevant practice and/or advanced nursing practice
Desirable criteria
  • Mentorship or teaching qualification / qualification in teaching or practice based learning
  • Independent Nurse Prescriber
  • Advance nurse practitioner qualification qualification
