An exciting opportunity has arisen for a Band 7 (1 year Fixed term) Physiotherapist within the Stroke Early Supported Discharge (ESD) team. The team provides rehabilitation for patients in their own homes in Hillingdon Borough.

An exciting opportunity has arisen for an enthusiastic and dynamic Band 7 Physiotherapist (1-year Fixed Term) to join the Stroke Early Supported Discharge (ESD) Team in Hillingdon. This is a rewarding opportunity to play a key role in delivering specialist stroke rehabilitation to patients within their own homes across the London Borough of Hillingdon, supporting individuals to regain independence and achieve their rehabilitation goals following stroke.

As part of a supportive and innovative multidisciplinary team, you will provide expert assessment, treatment and coordination of rehabilitation, helping to ensure seamless transitions from hospital to home. The role offers opportunities to lead service development, support junior staff and contribute to delivering high-quality, patient-centred stroke care in the community. This is an exciting time to join our service as we continue to develop and enhance community stroke rehabilitation pathways to improve outcomes for residents.

Main duties of the job

You will provide evidence-based rehabilitation in physiotherapy patients, often presenting with stroke needs, within people’s homes and supporting them to access the local community. You will supervise B6 and B4  staff within the community physiotherapy team, providing expert leadership and management. The post holder will have good communication and interpersonal skills. They must work as part of a multidisciplinary team and have excellent organisational and time management skills, teaching skills and an enthusiasm for service development.

The band 7 ESD co-ordinator will undertake a lead role in planning, co-ordinating, delivering and evaluating the caseloads on a day - day -basis. They will work closely with the Community Rehab team and local stroke units.

  • Car driver/ability to travel in community settings (commonly required)

Education & Qualifications

Essential criteria
  • Registration with HCPC as an Physiotherapist
  • Degree/ Diploma Physiotherapy
  • Evidence of relevant post graduate courses/ CPD
  • Member of specialist interest group
Desirable criteria
  • MSc , management course or equivalent level of study

Previous Experience

Essential criteria
  • Specialist stroke experience
  • Awareness of NSF long term conditions and NICE guidelines
  • Experience of teaching, supervision and mentoring of students of physiotherapy and other AHP’s.
  • Experience of spasticity and orthotics management
  • Experience of splinting and specialised seating
  • Experience of audit/policy/research.
  • Experience of performance management and appraisal.
  • Strong concept of case management role
Desirable criteria
  • Specialist community experience Experience of FES (Functional Electrical Stimulation

Skills and knowledge

  • Comprehensive clinical knowledge and skills encompassing a wide range of different complex conditions including stroke.
