Bank - Community Learning Disability Nurse / GP Link Worker

Job overview
We are seeking an enthusiastic and motivated Bank - Band 6 Community Learning Disability GP Link Worker to join our Bank, to initially support the Redbridge Community Learning Disability Team. This is a specialist role designed to strengthen the links between GP practices and the Learning Disability service, supporting improvements in the physical health, wellbeing, and care planning for people with learning disabilities.
We are seeking a compassionate, skilled, and proactive professional who can foster strong relationships with GP practices and collaborate effectively across services. You should have:
A recognised Learning Disability nursing qualification or considerable experience in Learning Disability services
· Or experience working within a GP or Primary Care setting
· Strong skills in assessment, care planning, and risk management skills.
Main duties of the job
- Improve the quality of annual health checks for people with learning disabilities.
- Validate GP Learning Disability Registers.
- Increase the uptake and quality of Universal Care Plans.
- Undertake holistic assessments, care planning, and implementation of care.
- Complete Health Action Plans and Hospital Passports.
- Carry out physical health checks, including ECGs.
- Undertake risk assessments and develop risk management plans.
- Work in close partnership with the multidisciplinary team (MDT), GPs, and Primary Care colleagues.
