Job overview

JOB PURPOSE

The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies, and the locality teams to deliver excellent standards of patients care for patients requiring support.


To provide skilled nursing care working under the supervision of the unplanned care advanced nurses and paramedics.


The post holder will undertake unplanned care community nursing duties which involve delivery of nursing services and holistic assessments of patients’ health needs using a person-centred approach.


Work shift patterns including weekends and bank holidays. The service operates through pre-rostered shift patterns spanning 8am – 10pm 7 days per week. 


 The post holder will provide quality and seamless nursing care to housebound patients and be proactive in coordinating their care needs as appropriate, whilst avoiding unnecessary admission to hospital or long-term care institutions. In addition, the post holder will be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery. 

Main duties of the job

To give skilled nursing care, based on principles of best practices and clinical evidence which includes, 
-    Catheterisation
-    Phlebotomy
-    IV Administration
-    S/C Injections
-    Palliative support


Act as a named nurse with the support and supervision of the unplanned care specialist clinicians. To take continued responsibility for the holistic health needs assessment including referring to appropriate services for ongoing management. E.G District planned care nurses. 

Act as the patients’ advocate and to facilitate the patients’ own choices with regards to nursing care, promoting independence and self-care as appropriate. 


To identify and record new problems and report back to unplanned care advanced nurses and refer as appropriate to other MDT for ongoing management. 

Maintain accurate, comprehensive, and contemporaneous records in accordance with trust and NMC guidelines. 

To provide information and support to patients, relatives and other carers that promotes and optimises positive health.

Skills and Knowledge

Essential criteria
  • Knowledge and evidence of catheterisation skills.
  • Ability to risk assess a situation and act appropriately
  • Ability to communicate effectively both verbally and in writing
  • Basic IT skills including use of Microsoft Office package and email
  • Knowledge of out of hospital care and the role of community unplanned care nursing.
  • Ability to listen effectively and ability to give constructive feedback to patients and colleagues
  • Knowledge of general wound care
  • Knowledge and evidence of phlebotomy.
  • Knowledge and evidence of IV administration skills
  • • Principles of care for patients with long term conditions, relevant NSF’s and other national standards
Desirable criteria
  • Knowledge of recent government legislation and changes in community care delivery
  • Understanding of resource management
  • Knowledge of the audit process

Experience

Essential criteria
  • Recent clinical nursing experience negotiating and liaising within a multidisciplinary team
  • Working with people with complex health needs
  • Experience of undertaking general assessment by utilising evidence based care.
  • Previous experience in the Nursing process, care planning and fluent in record keeping
  • Experience of providing holistic care that involves the MDT.
Desirable criteria
  • Experience in training other students.
  • Supporting students and other learners
  • Participate in auding, research and service development
  • Post registration experience in the community environment.

Education/Qualification

Essential criteria
  • Registered Nurse with current NMC registration.
Desirable criteria
  • Evidence of other post-registration education and training

Personal/Physical Attributes

  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
