Are you an experienced Diabetes Nurse looking for your next opportunity within community diabetes care?

An exciting opportunity has arisen for a Band 6 Community Diabetes Specialist Nurse to join our Brent Community Diabetes Team on a 9-month fixed-term contract to cover maternity leave.

Working as a key member of our supportive multidisciplinary team, you will help deliver high-quality, integrated diabetes care to patients within the community.

You will provide specialist assessment, treatment and ongoing support for people living with diabetes, working closely with patients, families, healthcare professionals and partner services to promote effective diabetes management and positive health outcomes.

The role will include delivering care within community settings and patients’ homes, maintaining high-quality clinical records and supporting the delivery of diabetes education to healthcare professionals.

You will also have the opportunity to contribute to service development, working with colleagues and service users to develop and evaluate evidence-based diabetes interventions that respond to the needs of the local community.

Main duties of the job

  • Provide specialist diabetes care to adults receiving oral hypoglycaemic agents, insulin, insulin conversion and other therapies across GP practices, health centres and patients’ homes.

  • Use clinical expertise and professional judgement to assess complex patient needs, develop evidence-based treatment plans and adapt care in response to changes in clinical condition.

  • Provide specialist diabetes advice, treatment, counselling and lifestyle support to adults with complex and diverse healthcare needs, promoting positive behavioural change and effective self-management.

  • Provide expert diabetes advice and clinical guidance to medical practitioners through virtual clinics, email and telephone consultations.

  • Work collaboratively with the multidisciplinary Community Diabetes Team and partner agencies, contributing to MDT meetings and case conferences to ensure coordinated and continuous care.

  • Use clinical judgement to identify when patients require additional support and make appropriate referrals to other healthcare professionals, specialist services and Social Services.

Education/Qualification

Essential criteria
  • Degree in Nursing or equivalent.
  • Current registration with the NMC, Showing a registered General Nurse.
  • Relevant post registration training in Diabetes eg: diploma in diabetes management.
Desirable criteria
  • Mentorship training.
  • Supervisory skills training.
  • Post registration training in Cognitive Behavioural Therapy

Experience

Essential criteria
  • Experience in Diabetes.
  • Experience in managing a clinical case load.
  • Experience in the training of students.
  • Excellent group education and presentation skills
  • Experience of conducting audits and/or research.
  • Experience in supervising others.
  • Experience working in a multi-disciplinary team
Desirable criteria
  • Experience working in Community settings.
  • Experience with working with change behaviour techniques.

Skills & Knowledge

Essential criteria
  • Ability to undertake leadership role in clinical speciality.
  • Advanced skills in the assessment and treatment of clients with diabetes
  • Advanced knowledge of evidence-based practice associated with diabetes.
  • Ability to interpret health and social policies and guidelines and to translate into practice.
  • Ability to problem solve, prioritise and demonstrate clinical reasoning based on experience and knowledge.
  • Highly developed communication and report writing skills, in verbal and written format.
  • Highly developed presentation skills and ability to present to professionals at all levels.
  • Ability to manage time effectively, meet deadlines and deal with work pressures and demands.
  • Basic IT skills – Microsoft Word, Power point, Excel.
  • Knowledge of and commitment to equal opportunities
  • Ability to work with clients from a diverse range of backgrounds

  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
