To provide high quality effective physiotherapy assessment and short term treatment/management programmes for adults presenting with a range of conditions in Haringey, who require a community based rehabilitation service.
To use specialist physiotherapy knowledge and skills to manage own complex caseload, as an autonomous practitioner.
To act as a source of expertise on falls, orthopaedic and community rehabilitation liaising closely with specialist services; providing advice to patients, carers, all grades of physiotherapists, other health care professionals, consultants, general practitioners and social services.

To deliver patient care based on principles of best practice, achievable through direct patient contact or via supervision.
To carry out specialist, comprehensive assessments of patients with a complex presentation, using investigative and analytical skills formulating individualised management and treatment plans.
To provide specialised physiotherapy interventions at home or in a community setting as required, collaborating with colleagues in the community and secondary care health professionals to promote continuity of care.
To identify own learning needs related to clinical knowledge and skills in the management of patients following stroke and with long term neurological conditions.
To support in the delivery of multidisciplinary education and training.
To access further education programmes to develop knowledge and skills, and to share learning with the wider team.
To support in the monitoring of the effectiveness of the community therapy service through the collation of data for ongoing audit.
Contribute to the evaluation of research findings and disseminate them appropriately, including presentations at study days or conferences.
To adhere to Whittington Health policy and procedures at all times.
To be aware of one's own continuous professional development needs and maintain an annual personal development plan.

- At least 2 years post-registration experience, including rotational
- experience and evidence of dedicated clinical practice and post-graduate

- Knowledge of outcome measure tools relevant to community Physiotherapy and rehabilitation, and ability to select and justify their use
- Demonstrate an understanding of responsibilities under the Health and Safety at Work Act and of assessing risk within clinical work.
