The Sister/Charge Nurse will be based in Tameside and Glossop Integrated Care NHS Foundation Trust, Acute Frailty Hub .
The post holder is responsible for leading and managing the nursing team, ensuring high standards of care for older people living with frailty. This role includes coordinating patient assessments, overseeing the implementation of care plans, and ensuring that nursing practices align with best practice.
Key duties include supervising junior staff, providing mentorship and training, and facilitating effective communication within the multidisciplinary team. The post holder plays a crucial role in triaging patients, managing clinical workflows, and ensuring timely interventions. Additionally, they are involved in quality improvement initiatives, patient safety protocols, and the evaluation of care outcomes, all while advocating for the needs for service users and their families. Overall, this post holder requires strong leadership skills, clinical expertise in nursing, and a commitment to delivering compassionate care.

Education and Qualifications
Essential criteria
- RGN with current NMC registration
- ILS
Desirable criteria
- Working towards a degree in a relevant healthcare topic
- Ability to travel
- Level 6 Teaching & Mentoring qualification.
Experience
Essential criteria
- Significant experience in managing acute conditions.
- Extensive clinical knowledge in the care and management of the acute / complex conditions, Frailty, Palliative care & intermediate care
- Proven Track record in acute care supporting Adults.
Desirable criteria
- Evidence of continued leadership and management of staff and service.
- Experience of managing and delivering sustained change.
Skills and Knowledge
Essential criteria
- Knowledge of Professional Scope of Practice.
- Knowledge of record keeping.
- Knowledge of health and safety.
Desirable criteria
- The post holder will be required to exert frequent moderate physical effort for several short periods during a shift e.g. moving patients over 15 kilos with mechanical aids.
- Frequent concentration on patient assessment and responding to frequent changing needs in the clinical area.
