As a vital link between primary and secondary care, our mission is to prevent unnecessary hospital admissions and enhance life quality for patients. You'll be at the forefront of transforming diabetes management—offering essential support, expert advice, and comprehensive education not only for patients but also for their families, carers, and fellow health professionals.

Based at the modern Moorgate Primary Care Centre in Bury and the Phoenix Centre in Heywood, our community-focused team delivers tailored diabetes interventions in various settings—ensuring that our patients receive the personalised care they deserve.

Join Our Team as a Diabetes Specialist Nurse

Are you an enthusiastic first-level nurse with a passion for diabetes care ? We're looking for a dynamic candidate to join our innovative team for a 9 month fixed term contract to cover maternity leave, where your expertise can truly shine.  Full and part time hours available.  

You’ll bring your current NMC registration and up-to-date knowledge of the diabetes speciality, along with experience in managing this complex condition. You’ll have the opportunity to initiate and adjust injectable and oral diabetes medications—empowering patients to manage their health more effectively.

Essential Qualifications/Experience

  • Registered Nurse with current NMC registration
  • Evidence of professional development at Certificate/ Diploma/ Degree level in adult Community Nursing e.g. District Nurse, Practice Nurse or Community Nurse
  • Mentorship/teaching or coaching qualification

  • ENB 928/Warwick Diabetes Course or an equivalent
  • National Independent Prescribing qualification V300 or willingness to undertake training

Knowledge, Skills and Experience

  • Evidence of Continuing Professional Development
  • Assessment and analysis of complex needs of patient with diabetes.
  • Ability to advise on appropriate interventions, monitor, evaluate and adapt diabetes treatment as required.
  • Ability to develop educational resources that are evidence based to meet the needs of patients and carers
  • Willingness to acquire new skills & knowledge & undertake further training in line with the demands of the service provision
  • An ability to demonstrate a range of communication styles to various stakeholders
  • Good time management & team working skills
  • Good oral & written communication skills to related at all levels with patients, colleagues and members of the MDT
  • Ability to work independently
  • Ability to practice using behaviour change methods
  • Ability to deliver group education using a range of teaching techniques
  • Able to reflect and appraise own performance
