Job overview

The Bury Community Mental Health Team are looking to recruit a dynamic and experienced Community Mental Health Practitioner.

We are keen to hear from enthusiastic and passionate professionals (including RMN/RN/Social Worker/OT) and are able to offer an exciting opportunity for a Band 6 within a well established Community Mental Health Team.

The successful candidates will work both autonomously and as a part of an MDT providing timely access to specialist services for working age adults with complex mental health needs.

The post will be based in Bury but there is an expectation that you will be able to travel across The Trust footprint.

Main duties of the job

  • To act as named practitioner for a caseload of allocated clients.
  • Take the lead in identifying risk and implementing safe and effective risk management strategies collaboratively.
  • To support and empower clients through recovery focused collaborative care-planning.
  • To work with the wider MDT to provide safe effective care in a recovery focused way.
  • To ensure all documentation is relevant and up-to-date in-line with NMC/SWE/HCPC standards.
  • To ensure safe and supportive discharge from inpatient services in line with The MHA (1983).
  • To contribute to case formulation and treatment plans.
  • To contribute to service development in collaboration with the wider team.
  • To participate in the education programmes of nursing students and other disciplines.
