Job overview
To be responsible for the tracking and recording of patient diagnostic tests on the ATR PTL (Awaiting Test Results Patient Treatment List), weekly monitoring and regular updating of the patient action plans on the hospital database. You will need good communication skills and be able to liaise closely with clinicians, other hospital departments and colleagues.
Main duties of the job
· Responsible for the collection of information required to support the monitoring of patients on clinical pathways. This involves using a variety of systems and liaising with a number of departments within the hospital to obtain the relevant information.
· To ensure that complete, accurate and timely data is recorded on the Patient Administration System and other Trust systems as required in line with local policy.
· To liaise with other departments ensuring relevant information is communicated in a timely manner.
· To communicate patient progress through a pathway regularly and effectively to the relevant clinicians and managers.
· To intervene when patients are at risk of breaching the referral to treatment / ATR target.
· To escalate to managers when patients are at risk of breaching the referral to treatment / ATR target.
· Appropriate use of relevant information technology systems e.g. Patient Centre, Word, Excel, PowerPoint, Digital Dictation etc. Create and maintain databases or spreadsheets as required.
· To ensure all correspondence is available for clinics, patient admissions and investigations.
· To deal with potentially distressing information and correspondence in a professional manner.
· Any other duties commensurate with the grade and in line with the requirements of the post.

Detailed job description and main responsibilities
To be responsible for the tracking and recording of patient diagnostic tests on the ATR PTL (Awaiting Test Results Patient Treatment List), weekly monitoring and regular updating of the patient action plans on the hospital database. You will need good communication skills and be able to liaise closely with clinicians, other hospital departments and colleagues.
You will also be responsible for managing the patient’s pathway by responding to queries and requests in clinical correspondence, communicating these actions via the use of IT solutions provided to communicate with the Consultant Secretaries and Waiting List Officers. The role and duties must be carried out in line with all national and local targets and all KPI’s achieved.
