A great opportunity has arisen for a Health Care Support Worker to join our community Palliative Respite Team.

The post holder will work as part of the Palliative Care Respite Team and will be responsible to the Team Coordinator. The Post Holder will assist the District Nursing Team in the delivery of high quality community based nursing and social care to patients who have palliative care needs and who wish to remain at home. The post holder will work collaboratively with other members of the Intermediate Tier Services in supporting the patient and carers and provide personal, social, psychological and spiritual care. They regularly work without direct supervision in a patient's home. 

Main duties of the job

  • Good interpersonal skills/communication (verbal, written, non-verbal)

Experience

  • Experience of working within a health or social care setting

Qualifications

  • NVQ Level 2 or 3 in care or the equivalent level of skill and knowledge gained through experience and competence assessment
  • Appreciation of community care philosophy and procedures gained through formal qualification or experience

Knowledge

  • Awareness of National and Local policies regarding Palliative and End of Life Care
  • Knowledge of community care and the delivery of care to patients in different environments
