The post involves working within the Central Manchester Intermediate Care Service delivering holistic assessments & interventions to patients in Willow bank Lodge Intermediate Care Unit & their own homes/ place of residence. Independent travel within the local geographical area to deliver the role is essential.

The Central Intermediate Care service operates  7 days per week. Therapy provide weekend cover on a Rota basis. 

Hours of work are; Early shift 07:30-15:30, Late Shift 10:00-18:00

With the above in mind, you must have experience of working in the NHS and in community services.  The successful candidate will be able to demonstrate an appropriate level of clinical skills in occupational therapy, will be experienced in administering person-centered assessments, as well as implementing and evaluating occupational therapy interventions in a fast paced working environment. You must have a recognized Occupational Therapy qualification and current HCPC registration.

Main duties of the job

The individual will manage a clinical caseload of patients, including those with complex and highly complex needs. You will supervise the band 5 and Band 3 support staff with caseload management.

The post holder will work without direct supervision as a lone worker in the community assessing, planning, implementing, evaluating and modifying individualised treatment programmes  for patients with low to highly complex needs.

The post holder will work as a team member collaborating with colleagues to ensure patient needs and goals are met using approaches which promote choice, participation and quality of life.

The post holder will ensure patients are triaged and managed on appropriate and timely pathways of care by liaising closely and working in partnership with other health and social care professionals including GPs, consultants, specialist equipment agencies, social service partners and voluntary agencies. You will support patient flow through supporting the timely discharge of patients from the acute setting.

The post holder will provide complex professional and clinical advice to patients, family members, carers and health and social care professionals including GPs, therapy colleagues and care managers.
