Job overview

We have an exciting opportunity for a self-motivated, experienced
Occupational Therapist to join our Intermediate Care Therapy Team in Oldham Community. 
We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries supporting the NHS Long Term Plan, providing rehabilitation and safe discharge home.

The post holder will work as part of the multi-disciplinary team, providing rehabilitation and planning discharges.  They will work in line with the agreed strategic objectives of Northern Care Alliance.


The aim of the team is to provide holistic rehabilitation and timely discharge by promoting independence and a strength based approach.  The team works also works closely with community services to support the home first model.

Main duties of the job

The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team within Intermediate care.  The candidate will be able to demonstrate strategic planning and have a strong clinical background in intermediate care/rehabilitation or a similar field.  Excellent time management and advanced communication skills are required along with awareness and understanding of local, regional and national agendas in respect to Health & Social care.

Essential criteria
  • Occupational Therapy

  • HCPC registration
Desirable criteria
  • RCOT or CSP registered

Knowledge

Essential criteria
  • Previous experience within intermediate care/Rehabilitation
  • Experience of management and / or leadership
  • Conduct comprehensive therapeutic assessment including assessing for mobility aids / stairs / equipment / dressing aids
  • Ability to effectively discharge plan complex patients
  • Able to manage own time effectively including planning and prioritisation of both clinical caseload and leadership responsibilities
Desirable criteria
  • Experience of participating in clinical audit
  • Engaged in clinical supervision
