Job overview

We are seeking to recruit an Advanced clinical Practitioner to work within an exciting new service development within Pennine Care. We are looking for individuals who are passionate about working with children and young people and working innovatively across pathways and traditional boundaries to ensure that young people get the right care, from the right clinician/team, at the right time and in the right place in order to improve their outcomes and the families/carers experience of mental health support.

This is an exciting opportunity for a qualified advanced clinical practitioner to work alongside the highly skilled existing team consisting of band 6 MHPs, band 7 SMHP /team leaders, youth support workers, an Operational manager, and an Advanced clinical practitioner. The successful candidate will offer clinical leadership, advanced level of assessments and risk formulations for CYP seen in the crisis team. They will have responsibilities for the training needs of the workforce and provide clinical supervision to those providing assessment, support, signposting as well as brief therapeutic intervention where appropriate to young people who required crisis support within our CYP crisis teams which include Home Treatment, Rapid Response and Enhanced Rapid Response-pediatrics liaison within the Pennine Care footprint.

 

 

Main duties of the job

Main Duties and Responsibilities

Responsibility for Patient Care (including monitoring, diagnostics, investigations, assessment, formulation)

As a member of the CAMHS care hub to act as an autonomous practitioner providing clinical assessment, diagnosis (where possible), and treatment (where appropriate) for patients referred to the urgent and acute pathways within their scope of professional practice.

To be responsible for managing referrals including the initial assessment request and interpret investigations and propose differential diagnoses for children with complex mental health conditions.

To offer clinical supervision, reflective practice and complex case management support to the senior mental health practitioners and Team Manager within the care hub.

Detailed job description and main responsibilities

To provide advice, consultation and liaison to CAMHS teams and wider children’s services regarding referrals, signposting and appropriate care and treatment

To triage and prioritise children and young people referred to the Acute Pathway demonstrating the use of a variety of techniques to elicit a comprehensive history to determine an appropriate care plan.

To identify changes in a patient’s condition through clinical examination and take appropriate action to ensure the safety of the patient.

To undertake interventions consistent with evidence-based practice, transferring and applying knowledge and skills to meet the child or young person’s needs.

To use the skills and knowledge to make referrals for diagnostic tests and instigate investigations to support diagnosis if required

To evaluate the effectiveness of interventions in meeting prior agreed goals and making any necessary modifications.

To undertake necessary assessments and as a non-medical prescriber, prescribe within the scope of professional practice.

To ensure that the care provided and services delivered are in line with local policies and procedures and the Clinical Governance Framework.

To maintain accurate and contemporaneous records in accordance with local and national professional policies and guidelines.

To use advanced clinical skills and expert knowledge to provide proactive monitoring and provide timely intervention.

Person specification

Essential

Essential criteria
  • Relevant Core Professional qualification in mental health e.g. registered nurse, clinical psychologist, social worker plus evidence of continued Professional development in a HYM related area
  • Master Level qualification, minimum PGD level advance clinical practice.
  • Advanced Nurse practitioner Qualification
  • V300 Non-medical prescribing qualification or willingness to work towards
  • • Demonstrate significant post- registration experience at senior grade • Extensive experience of mental health nursing ideally in a child and adolescent setting
  • Extensive experience of mental health nursing ideally in a child and adolescent setting
  • Professional development and clinical expertise at Masters Level.
  • Up to date professional portfolio demonstrating extensive evidence of reflective practice.
  • Evidence of experience working collaboratively with other disciplines / agencies and supervision of others
  • An understanding of systems in health and social care that is wider than their immediate role.
  • Excellent risk assessment and formulation skills
  • Extensive clinical knowledge of complex neurodevelopmental and child and adolescent mental health conditions and their management.
  • First contact initial assessment and diagnostic skills
  • Extensive knowledge of safeguarding with particular evidence of children with complex mental health conditions.
  • Excelle• Knowledge and understanding the principles of governance, particularly in relation to their service area.nt leadership skills
  • Excellent time management and ability to work under pressure.
