This is an excellent opportunity for a highly motivated and enthusiastic Band 6 District Nurse  to join the Adult Community Nursing Team, working flexibly over a seven day period.  The Specialist Practitioner Qualification in District Nursing  is desirable or at least substantial years experience working at band 5 level  within the district nursing service. 

The Broughton  District Nursing  Team delivers care between the hours of 08:00 - 18:00  in the community of Salford to people in their own homes. We are part of a dynamic, integrated health and social multidisciplinary team delivering high quality care, advice, and support and monitoring to patients, their families and staff within the community settings. You will work under the direction and supervision of the District Nurse Lead to provide care to patients with complex and non-complex needs with the aim of promoting independence and self-management.

You should have excellent communication skills.

You’ll be supported by an expert team in an environment where training and development opportunities are actively encouraged in order to meet the clinical needs of the role. We offer a comprehensive induction programme and you will be appraised on a yearly basis with a view to developing your knowledge and skills where appropriate.

To provide nursing care to patients within the community environment and plan the workload for junior staff and support the team leader. 

To participate in the delivery of nursing care and support to clients within the community setting and have the necessary confidence and competence to work autonomously.

To work under the direction of the Team leader and to be responsible for assessment and management of the health needs of an identified population including end of life care and patients with long term conditions and/or complex nursing needs.

Communication and interpersonal skills are essential for the role as you will often be required to chair team safety huddles, attend multi professional meetings, discharge planning meetings etc. and to feedback to the team to ensure seamless patient care.

The post holder is responsible for the assessment of patient care needs and development, devising, implementing and evaluating programmes of care, both for a specific group of patients and in supporting the care of all patients. As a Sister/Charge Nurse you will provide daily clinical and managerial leadership to nursing and support staff in the delivery of evidence based practice.
