Working alongside the ACPs and Medical team within Acute Medicine, and the wider multidisciplinary team, the post holder will deliver safe, timely and effective care for patients at risk of, or diagnosed with, VTE.

The role will include VTE prevention and risk assessment, assessment of suspected DVT/PE, anticoagulation management and patient education, alongside supporting ward teams and contributing to service development, audit and education.

The post holder will also contribute to the development and delivery of appropriate outpatient pathways for patients requiring ongoing management following a pulmonary embolism, working collaboratively with acute and community services to ensure continuity of care.

Provide patient education and develop outpatient pathways for ongoing management following pulmonary embolism, working with acute and community services.

Maintain accurate clinical records and monitor service effectiveness. Develop and implement VTE guidelines and pathways, participate in clinical audits and the Thrombosis Committee, and use NICE guidance to improve patient outcomes and safety.

Provide specialist advice to patients, families and healthcare professionals. Coordinate referrals, address concerns and support staff education. Demonstrate clinical leadership, identify training needs and work collaboratively with the multidisciplinary team to develop specialist practice and improve VTE care across the Trust.
