Stockport Urgent Response and Rehabilitation Team (SURRT) are pleased to be able to offer a 6 month fixed term contracts, full time posts for Senior Support Workers

We are a proactive and dynamic community team, providing care in the patients own home, working across Stockport to prevent avoidable hospital admissions and ED attendances and facilitate discharges from hospital. You will be expected to drive and have access to a vehicle.

You will be part of a multi professional team, undertaking higher level nursing, therapy and social care tasks within agreed parameters, delegated by the registered practitioners. The service aims to  support patients in an urgent or crisis situation to remain in their own homes and by facilitating early and supported discharge from hospital.

' You will be involved in all aspects of the assessment, treatment and discharge process, and work alongside both health and social care colleagues to ensure that patients remain as independent as possible and achieve the best possible health, therapy and social outcomes.

This is a challenging yet rewarding role in a fast paced environment which will give you the opportunity to utilise your senior support worker skills.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £43.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
