Fixed Term 12 Month Contract to 30th June 2022

An exciting opportunity to join a dynamic team and be part of Access and Intermediate care, delivering care in the community as part of the admission prevention and hospital discharge pathways.

Are you an experienced and enthusiastic person with experience in delivering care and looking to make a difference to the lives of people in your local community? Do you want flexibility with the opportunity to develop skills, knowledge and abilities to support people in your local community…if so your local NHS needs you!

This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable.  We are looking for candidates with experience of providing care in the community and working as part of a multi-disciplinary team.

You will be supported through training as part of induction and development in your role. You will be supported to complete the NHS Care Certificate and be provided with opportunities to develop and enhance skills. This role provides opportunities to participate within a multi-disciplinary team of health and social care professionals, and development within the wider Wirral Community NHS services.

You will be supporting people with activities of daily living, personal care, moving and handling, prompting medication, meal preparation and social care and support.  You will be supporting people to remain independent in their own homes and also in the period of transition following a short stay in hospital whilst awaiting for other home care services.

There are also opportunities for ward based positions within Clatterbridge.  Preferences  can be discussed at interview

required to work over a 7 day shift pattern 8am – 10pm Monday – Sunday
