Specialist Extended Rehabilitation Case Manager

Job overview

NHS Cheshire and Merseyside Pathfinder scheme will centralise the commissioning and case management arrangements for patients with complex extended rehabilitation needs across Merseyside and Warrington. The service will provide specialist support for patients, carers and colleagues across the region, ensuring patients are identified, transferred and case managed through the most appropriate pathway to meet their rehabilitation needs.

Main duties of the job

The role of the Specialist Case Manager is to provide a case management function for patients requiring extended rehabilitation within IS services.  This includes:

Provide specialist support for patients, carers and colleagues within acute and specialised NHS services who are identified as having extended rehabilitation needs.

Provide expert guidance on most appropriate pathways of care and placement options within IS providers.

Facilitate and coordinate assessments and costings by independent sector providers and source appropriate funding through NHS Cheshire and Merseyside.

Support appropriate and timely flow of patients to reduce levels of delayed transfer of care across the pathway of care.

Regularly monitor of patient progress against rehabilitation goals by attending multi-disciplinary team (MDT) meetings to discuss progress and support the discharge planning process. 

Build and maintain relationships with key stakeholders and individuals across NHS inpatient services, local authorities, community services, IS providers and All Age Continuing Care teams to ensure that a consistent and high-quality service can be provided.

Ensure that qualitative and quantitate data is accurately recorded to support the team leader in providing assurance to commissioners and development of a long-term strategy for case management across the region.

  • NMC Registered General Nurse, Mental Health Nurse
  • Allied Health Professional.

Knowledge and Experience

Essential criteria
  • Experience of working within a multi-disciplinary team (MDT) and chairing MDT meetings or panels.
  • Experience of working with patients with complex rehabilitation needs on an extended pathway.
  • Provision and receipt of highly complex, sensitive or contentious information, negotiating with stakeholders as required.
  • Presenting complex and sensitive information to a range of recipients.
  • Demonstrate the ability to perform to a high level in a clinical setting, holding services to account and ensuring effective use of NHS resources.
  • Knowledge and understanding of the philosophies of Safeguarding of Vulnerable Adults and children.
  • Detailed knowledge and understanding of the background to and aims of current healthcare policy and the implications of this on commissioners, providers and key stakeholders.
  • Knowledge of legal issues pertaining to complex care client group.
  • Commitment to a focus on quality and promotion of high standards to consistently improve patient outcomes.
  • Budget monitoring and working to Standard Financial Instructions.

Skills and Attributes

Essential criteria
  • Demonstrate effective problem solving skills and ability to respond to sudden unexpected demands.
  • Excellent negotiation skills with the ability to offer constructive challenge.
  • Ability to produce concise and accurate verbal and written reports in accordance with professional body guidelines and ICB policy and protocol.
  • Demonstrate sound IT skills.
