Main duties of the job
The role involves working as part of the multi-disciplinary team to maintain and expedite patient flow through the wards with the support of
Complex Discharge Nurses.
The successful candidate will liaise with external agencies and care providers, offer support to the team. The successful candidate would be
joining a supportive and enthusiastic team who are committed to providing high standards of patient centred care and timely and safe discharge.

Detailed job description and main responsibilities
To provide a single point of contact on a named ward/wards for patients, families, carers and associated people and co-ordinate/contribute to the
safe and timely hospital discharge in partnership with other multi-disciplinary colleagues.
Ensure that patient and family/carer have clear expectations regarding anticipated length of stay.
Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other members of the MDT;
doctors, Nurses, occupational therapists, physiotherapists and other hospital assessment and discharge team members.
Establish and maintain productive integrated working relationships with social service and community healthcare colleagues as required.
To develop strong working relationships with ward teams, Discharge Lounge, Therapy teams, Discharge co-coordinators pharmacy etc., to
support and co-ordinate timely and appropriate assessment and safe discharge of people from acute beds.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
