Job overview
North Sefton Planned Therapy service provides therapy and rehabilitation in patients homes and care home settings. We aim to promote independence and improve quality of life for people with physical problems caused by accidents, ageing, disease or disability. These include respiratory conditions, orthopaedic and musculoskeletal conditions, mobility issues, neurological conditions and general age related decline, where rehabilitation goals are identified.

We are looking for an individual experienced in community care, who has experience of managing patient with multiple and complex long term conditions.
We are seeking an individual who has experience supervising both staff and students, and looks to develop the workforce. If you are passionate about continuous development and have contributed to service development, we would like you to apply.
We’re a very supportive team with physiotherapists, occupational therapists, rehab assistants and assistant practitioners who work closely with other members of the MDT.
Main duties of the job
The post holder will be expected to effectively manage a caseload of patient’s with multiple and often complex co-morbidities as an autonomous practitioner, using evidence based patient centred principles to assess, plan, implement and evaluate interventions of the patient group.
The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal. The post holder will participate in planning, development and evaluation of services provided, holding responsibility for defined service improvement projects. The post holder must have experience supervising junior staff and students.
The post holder will take a holistic approach in the assessment and treatment of patients, being responsible for the provision of appropriate, high quality therapy and care to a caseload which may be of a complex nature, using the appropriate model of care and evidenced based practice.
The post holder should be able to complete a comprehensive assessment, including the appropriate use of outcome measures and generate a goal focused treatment plan.

Detailed job description and main responsibilities
The post holder will be expected to effectively manage a caseload of patient’s with multiple and often complex co-morbidities as an autonomous practitioner, using evidence based/ patient centred principles to assess, plan, implement and evaluate interventions of the patient group.
To prioritise designated referrals according to need, risk and service capacity.
To plan and implement patient centred individual and/or group interventions, using graded activity to achieve therapeutic goals including sensory integration difficulties within rehab hub setting.
To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention.
To ensure that the service users care plan is developed to meet the assessed need of the individual. To monitor, via performance management, the care plans developed for service users by all sector members to ensure that they are appropriate and of a high standard.
To liaise with family members, carers, GP’s and other professionals as necessary when completing the assessment and developing a care plan.
Understanding risk assessment, being able to assess risk and complete management plans.
The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal.
The post holder will participate in planning, development and evaluation of physiotherapy services within a designated area/team, holding responsibility for defined projects.
The post holder take a holistic approach in the assessment and treatment of service users / patients, being responsible for the provision of appropriate, high quality care to a defined caseload which may be of a complex nature, using the appropriate model of care and evidenced based practice.
Work in partnership with other professional's to enable patients / clients to be maintained in an appropriate environment, thus avoiding hospital admission.
To contribute to the maintenance and development of the North Sefton Planned Therapy Team and facilitate effective learning within the area of practice for all students and practitioners.
