Job overview
This a 12 month fixed term position.
An exciting opportunity has arisen for an enthusiastic and motivated pharmacist to join the "Your Medicines @ Home" medicines optimisation team. The post holder will provide leadership for a dedicated team of pharmacy technicians and staff that directly support patients to use their medicines safely and remain independent at home.
The post holder will also work in a team of pharmacists and pharmacy technicians providing expert medicines advice to the primary care Community Health and Wellbeing multi-disciplinary teams.
Collaborating with health & social care colleagues across primary and secondary care, the post holder will deliver patient focused care through innovation and commitment.
Main duties of the job
You will possess excellent communication and organisational skills, be self-motivated and able to innovate through new ways of working. The ability to work effectively as part of a team and provide ongoing leadership to colleagues are also essential for this role.
The main duties of the role will include:
- Undertaking assessments of patients referred into the Your Medicines @ Home service and Community Health and Wellbeing teams, identifying medicines related issues and implementing appropriate actions to support independence and effective care.
- Provide guidance and support on complex medicines management issues to health & social care staff involved in the care of patients.
- Provide leadership and support to the Your Medicines @ Home team, managing demands through effective team working and service innovation.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Knowledge/Qualifications/Training
Essential criteria
- Master's degree in pharmacy, or equivalent
- Registered member of the GPhC
- Clinical diploma, or equivalent clinical experience
- Knowledge/experience of primary/secondary care transfer issues
- Knowledge/experience of supporting medication adherence in the community
- Knowledge/experience of medicines related issues in elderly patients
Desirable criteria
- Active membership of Royal Pharmaceutical Society
- Independent prescriber
- Understanding/experience of quality improvement methodology
Experience
Essential criteria
- Relevant post-qualification experience in a relevant setting
- Experience of audit/research
- Experience of working with multi-disciplinary teams
- Experience of managing service change / development
