The post holder will support the Pharmaceutical Service and the Medicines Management agenda in the UHB. They will be responsible for:

·        Assisting in the delivery of clinical pharmacy services to clinical areas to ensure safe, therapeutic and cost-effective use of medications. 

·        Ensuring patients are prioritised appropriately and high-risk patients are identified and referred to the appropriate member of the multi-disciplinary team or managed by the post holder.

·        Continuing the development of pharmacy services provided across the wards. 

Main duties of the job

The post holder will support the Pharmaceutical Service and the Medicines Management agenda in the UHB. 
 

The post holder will sit on the Medicines Management panel, assisting in the training and assessing of technicians, trainee pharmacists and new pharmacists in the process of ward-based working, including medicines reconciliation and discharge process. In addition, they will support others in developing clinical prioritisation skills.

The post holder will dispense, undertake technical check and issues prescriptions, providing specialist advice and information on medicines to patients, relatives, carers and nursing staff.

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Qualifications

Essential criteria
  • Demonstrable expertise and knowledge of dispensing and checking practices, to Pharmacy Degree Level or equivalent acquired through BTEC in Pharmaceutical Science, NVQ Level 3 in Pharmacy Services.
  • National Accreditation for Checking Pharmacy Technicians. Medicines Management accreditation.
  • Registered as a Pharmacy Technician with GPhC
  • Good working knowledge of pharmacy procedures and practice.
  • Current knowledge of hospital pharmacy practice.
  • Basic knowledge of clinical pharmacy.
Desirable criteria
  • Management qualification.
  • PWDS Qualification in Clinical Prioritisation or equivalent.
  • Endorsing standards qualification or equivalent.
  • BTEC Level 4 Professional Diploma in Pharmacy Clinical Services or equivalent.

Experience

Essential criteria
  • Significant post qualification pharmacy experience including working as a Pharmacy Technician.
  • Experience of training, mentoring and assessing staff.
  • Ability to know when to refer to a Pharmacist.
  • Ability to manage the day to day provision of pharmacy service within an area.
Desirable criteria
  • Experience working within surgical wards.
  • Experience of supervising/managing and organising ward based medicines management technician teams.

  • Good numeracy and literacy skills including; calculations, percentages, decimal, fractions.
  • Ability to manage a team and work constructively with other disciplines.
Desirable criteria
  • Welsh Language Skills (Level 3 and above/B2) are Desirable for this Role.
